结合压缩感知技术的自由呼吸单次激发电影序列在心律失常患者中的应用

Application of Free Breath Single-shot Cine Combined with Compressed Sensing Technology in Arrhythmic Patients

摘要 目的 比较压缩感知技术的自由呼吸单次激发电影(FB single-shot CS cine)序列与传统平衡稳态自由进动电影(bSSFP cine)序列对心律失常患者心脏磁共振(CMR)中的应用价值。方法 使用1.5 T MR对26例心律失常患者(男性17例,女性9例)分别采用bSSFP cine和FB single-shot CS cine序列进行CMR扫描,按照上述序列分为A、B两组。由两名观测者分别对心脏电影图像质量进行5分制主观评分,并计算心功能参数,包括左心室射血分数(LVEF)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、每搏输出量(SV)、心输出量(CO)和心肌质量(MASS)。测量血池和心肌的信号强度及背景噪声,计算心肌信噪比(SNR)、对比噪声比(CNR)。分别使用Wilcoxon检验以及卡方检验对两组序列心脏标准化参数、心肌CNR及图像质量评分结果进行组间统计学分析。结果 观察者数据一致性良好[组内相关系数(ICC)均>0.75]。B组4.450±0.583图像质量主观评分高于A组4.039±0.824(P=0.038)。A组与B组图像中心基底部、中央部、心尖部的SNR和CNR差异没有统计学意义(P均>0.05),LVEF、LVEDV、LVESV、SV、CO和MASS差异均无统计学意义(P均>0.05)。A组扫描成功率为74.286%,B组扫描成功率为100%。bSSFP cine扫描时间显著缩短约91%结论 FB single-shot CS cine序列能有效提高图像质量,并且显著缩短扫描时间,对于心律失常患者的CMR扫描时有较好的临床应用前景。 Objective To compare the free breathing single-shot compressed sensing cine(FB single-shout CS cine)with balanced steady state free precession cine(bSSFP Cine),The application value of FB single-shout CS cine sequence in cardiac magnetic resonance(CMR)in patients with arrhythmia.Methods A total of 26 patients with arrhythmia(17 males and 9 females)were scanned by 1.5 T MR Using bSSFP cine and FB single-shot CS cine,respectively,and were divided into groups A and B according to the above techniques.The quality of cardiac film images was subjectively scored on a 5-point scale by two observers,and calculated cardiac function parameters,including left ventricular ejection fraction,and the left ventricular ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV),stroke volume(SV),cardiac output(CO)and myocardial mass(MASS).To measure the signal intensity and background noise of blood pool and myocardium,and calculate myocardial CNR.Wilcoxon test and Chi-square test were used to analyze the results of standardized cardiac parameters,myocardial CNR and image quality scores of the two groups.Results The consistency of observer data was good(ICC>0.75).The subjective score of image quality in group B was 4.450±0.583,higher than that in group A(4.039±0.824,P=0.038).There were no statistically significant differences in SNR and CNR in image center base,center and apex between group A and group B(all P>0.05),and no statistically significant differences in LVEF,LVEDV,LVESV,SV,CO and MASS between group A and group B(all P>0.05).The component power of Group A was 74.286%,and the scanning success rate of group B is 100%.Conclusion Free-breathing single-shot CS cine sequence can effectively improve the contrast noise ratio of images,improve image quality,and significantly shorten the scanning time,and has a good clinical application prospect for CMR scanning in patients with arrhythmia.
