血清胆碱酯酶、肝素结合蛋白和微小RNA-127检测对重症肺部感染预后的预测价值

摘要 目的:探讨血清胆碱酯酶(S-ChE)、肝素结合蛋白(HBP)和微小RNA-127(miR-127)水平检测对重症肺部感染患者预后的预测价值。方法:选择重症肺部感染患者112例为观察组,另选取同期体检健康者100例为正常组,比较两组血清S-ChE、HBP、miR-127水平。根据预后情况将观察组患者分病死组和存活组,比较两组一般资料及血清S-ChE、HBP、miR-127水平。分析预后影响因素,以及血清S-ChE、HBP、miR-127水平对预后的预测价值。结果:正常组血清S-ChE水平高于观察组,HBP、miR-127水平低于观察组(均P<0.05)。本研究中,病死41例(病死组),存活71例(存活组),病死组急性生理与慢性健康评分(APACHEⅡ)、多器官功能障碍综合征评分(MODS)及血清HBP、miR-127水平高于存活组,氧合指数及血清S-ChE水平低于存活组(均P<0.05)。低氧合指数、低S-ChE、高HBP、高miR-127、高APACHEⅡ评分、高MODS评分为预后独立危险因素(均P<0.05)。血清S-ChE、HBP、miR-127对预后均有一定的预测价值,三项联合预测价值更高(均P<0.05)。结论:重症肺部感染患者血清S-ChE水平降低,HBP和miR-127水平升高,对预后均有一定的预测价值,且三项联合的预测价值更高。 Objective:To investigate the predictive value of serum cholinesterase(S-ChE),heparin-binding protein(HBP)and microRNA-127(miR-127)for the prognosis of patients with severe pulmonary.Methods:A total of 112 patients with severe pulmonary infection were selected as the observation group,and 100 healthy people in the same period were selected as the normal group.The serum S-ChE,HBP,and miR-127 levels were compared between the two groups.According to the prognosis,the patients in the observation group were divided into the death group and the survival group,and the general data and serum S-ChE,HBP,miR-127 levels of the two groups were compared.The influencing factors of the prognosis,and the predictive value of serum S-ChE,HBP and miR-127 levels for the prognosis were analyzed.Results:The serum S-ChE level in the normal group was higher than that in the observation group,and the levels of HBP and miR-127 were lower than those in the observation group(all P<0.05).In this study,41 cases died(death group)and 71 cases survived(survival group).The APACHEⅡscore,MODS score,serum HBP and miR-127 levels in the death group were higher than those in the survival group,and the Oxygenation Index and serum S-ChE level were lower than those in the survival group(all P<0.05).Low Oxygenation Index,low S-ChE,high HBP,high miR-127,high APACHEⅡscore and high MODS score were independent risk factors for the prognosis(all P<0.05).Serum S-ChE,HBP and miR-127 all had certain predictive value for the prognosis,and the combined predictive value of the three was higher(all P<0.05).Conclusion:The serum S-ChE level is decreased,while HBP and miR-127 levels are increased in patients with severe pulmonary infection,which all have certain predictive value for prognosis,and the combination of the three has a higher predictive value.
