专病特色管理模式在老年高血压患者中的应用效果

Application Effect of Specific Disease Characteristic Management Model in Elderly Patients with Hypertension

目的:探究专病特色管理模式在老年高血压患者中的应用效果。方法:选取2022年1—5月于昌平区崔村社区卫生服务中心门诊就诊的112例老年高血压患者作为研究对象,按随机数字表法分为两组,各56例。对照组实施常规管理模式,观察组实施专病特色管理模式。比较两组自我管理能力、血压水平、服药依从性、生活质量。结果:干预后,两组自我管理能力评分均较干预前高,且观察组较对照组高(P<0.05)。干预后,两组收缩压、舒张压水平均较干预前低,且观察组较对照组低(P<0.05)。两组服药依从性评分、生活质量评分均较干预前高,且观察组较对照组高(P<0.05)。结论:专病特色管理模式在老年高血压患者中的应用效果较好,可控制血压,提高患者的自我管理能力、服药依从性及生活质量。

Objective:To explore the application effect of specific disease characteristic management model in elderly patients with hypertension.Methods:A total of one hundred and twelve elderly hypertensive patients treated in the outpatient department of Changping District Cuicun Community Health Service Center from January 2022 to May 2022 were selected as the study objects and divided into two groups according to random number table method,with fifty-six cases in each group.The control group implemented the conventional management mode,and the observation group implemented the specific disease characteristic management mode.Self-management ability,blood pressure level,medication compliance and quality of life were compared between the two groups.Results:After intervention,the scores of self-management ability in both groups were higher than before intervention,and the observation group was higher than the control group(P<0.05);after intervention,the levels of systolic and diastolic blood pressure in both groups were lower than before intervention,and the observation group was lower than the control group(P<0.05);the scores of medication compliance and quality of life in both groups were higher than before intervention,and the observation group was higher than the control group(P<0.05).Conclusion:The application effect of specific disease management mode in elderly hypertensive patients is good,which can control blood pressure,and improve patients'self-management ability,medication compliance,and quality of life.
