雷火灸联合全息刮痧防治湿热下注型混合痔术后尿潴留的疗效观察

Efficacy of Thunder Fire Moxibustion combined with Holographic Scraping on Preventing and Treating Urinary Retention After Surgery for Mixed Hemorrhoids with Dampness-Heat Diffusing Downward Syndrome

目的:观察雷火灸联合全息刮痧防治湿热下注型混合痔术后尿潴留的价值。方法:选取2021年2月至2023年4月期间收治的110例混合痔术后患者,随机分为观察组和对照组,各55例,对照组采用全息刮痧治疗,观察组在对照组基础上采用雷火灸治疗。比较两组患者术后首次排尿时间、残余尿量及每日排尿量、尿动力学[首次尿意膀胱容量(FDV)、最大尿流率(Qmax)、残余尿量(RV)及逼尿肌压力]、并发症(尿路感染、肾积水、血尿、肾功能损伤)发生情况及舒适度[舒适状况量表(GCQ)]。结果:干预后观察组患者首次排尿时间短于对照组(P=0.001),残余尿量少于对照组(P=0.000),每日排尿量多于对照组(P=0.000);干预后观察组患者FDV、RV低于对照组,Qmax、逼尿肌压力高于对照组(P=0.000);干预后观察组患者GCQ评分高于对照组(P=0.000);观察组并发症发生率为1.82%(1/55),对照组并发症发生率为5.45%(3/55),两组比较,差异无统计学意义(P>0.05)。结论:雷火灸联合全息刮痧干预有利于改善湿热下注证混合痔术后尿潴留,可促进患者尿动力学恢复,且舒适度和安全性确定。

Objective:To observe the value of thunder fire moxibustion combined with holographic scraping on the prevention and treatment of urinary retention after surgery for mixed hemorrhoids with dampness-heat diffusing downward syndrome.Methods:A total of 110 patients after mixed hemorrhoid surgery from February 2021 to April 2023 were selected and randomly divided into observation group and control group,with 55 cases in each group.The control group was treated with holographic scraping,and the observation group was given thunder fire moxibustion on the basis of the control group.The postoperative first urination time,residual urine volume,daily urination volume,urodynamics[first desire to void(FDV),maximum urinary flow rate(Qmax),residual urine volume(RV)and detrusor pressure],occurrence of complications(urinary tract infection,hydronephrosis,hematuria,renal function injury)and comfort[General Comfort Questionnaire(GCQ)]were compared between the two groups of patients.Results:The first urination time in observation group was shorter(P=0.001),the residual urine volume was less(P=0.000),while the daily urination volume was more compared to control group(P=0.000).After intervention,FDV and RV in observation group were lower while the Qmax and detrusor pressure were higher than those in control group(P=0.000).The GCQ score in observation group after intervention was higher than that in control group(P=0.000).The incidence of complications in the observation group was 1.82%(1/55),while the incidence of complications in the control group was 5.45%(3/55),there was no statistically significant difference between the two groups(P>0.05).Conclusion:Thunder fire moxibustion combined with holographic scraping intervention is beneficial to improving the urinary retention after surgery for mixed hemorrhoids with dampness-heat diffusing downward syndrome,and it can promote the recovery of urodynamics of patients,and it has certain comfort and safety.
