经腹三维超声容积对比成像C平面诊断胎儿后颅窝囊性异常的价值及与胎龄的关系

Value of transabdominal three-dimensional ultrasound volume contrast imaging C plane in diagnosis of fetal posterior fossa cystic abnormalities and its relationship with gestational age

摘要 目的 分析经腹三维超声容积对比成像C平面(VCI-C)诊断胎儿后颅窝囊性异常的价值及与胎龄的关系。方法 选取2021年6月—2022年10月上饶市人民医院诊断的胎儿后颅窝囊性异常患者50例作为研究组,另选取同期进行健康体检的胎儿150例作为对照组,比较两组研究对象及研究组不同胎龄患者小脑蚓部前后径(AP)、头尾径(CC)、周长(C)及最大切面面积(MSA),脑干小脑蚓部角(BV),脑干小脑幕角(BT)之间的差异,研究经腹三维超声VCI-C诊断相关指标与胎龄的相关性。结果 研究组AP、CC、C、MSA、BV、BT分别为(14.52±2.33)mm、(16.92±3.61)mm、(42.15±3.17)mm、(588.35±9.33)mm^(2)、(31.18±2.09)°、(56.16±2.62)°;对照组AP、CC、C、MSA、BV、BT分别为(7.73±2.26)mm、(8.95±3.78)mm、(22.36±3.81)mm、(155.30±9.56)mm^(2)、(2.12±0.98)°、(29.22±2.22)°。与对照组比较,研究组患者的AP、CC、C、MSA、BV、BT均显著升高(均P<0.05)。与孕中期组比较,孕晚期组患者的AP、CC、C、MSA均显著升高(均P<0.05)。经腹三维超声VCI-C诊断与金标准的一致性较强(Kappa=0.526,P<0.05)。患者的胎龄与AP(r=0.562,P<0.05)、CC(r=0.441,P<0.05)、C(r=0.547,P<0.05)、MSA(r=0.582,P<0.05)均呈正相关。结论 经腹三维超声VCI-C诊断胎儿后颅窝囊性异常与现有的金标准结果一致,胎龄与VCI-C诊断指标呈现显著的相关性。 Objective To analyze the value of transabdominal three-dimensional ultrasound volume contrast imaging C plane(VCI-C)in diagnosis of fetal posterior fossa cystic abnormalities and its relationship with gestational age.Methods Fifty cases of fetal posterior fossa cystic abnormalities were selected from Shangrao People's Hospital from June 2021 to October 2022 as study group,150 cases of fetu-ses undergoing physical examination during the same period were selected as control group.The differences of vermis cerebelli anteroposterior diameter(AP),head and tail diameter(CC),circumference(C),maximum section area(MSA),brainstem and vermis cerebelli angle(BV),and brainstem and tentorium cerebelli angle(BT)were compared between the two groups of patients and the patients of different gestational ages in study group.The correlation between the diagnostic indexes of transabdominal three-dimensional ultrasound VCI-C and gestational age was analyzed.Results AP,CC,C,MSA,BV,and BT in study group were(14.52±2.33)mm,(16.92±3.61)mm,(42.15±3.17)mm,(588.35±9.33)mm^(2),(31.18±2.09)°,and(56.16±2.62)°,respectively;AP,CC,C,MSA,BV,and BT in control group were(7.73±2.26)mm,(8.95±3.78)mm,(22.36±3.81)mm,(155.30±9.56)mm^(2),(2.12±0.98)°,and(29.22±2.22)°,respectively.Compared with control group,AP,CC,C,MSA,BV,and BT in study group increased significantly(P<0.05).Compared with middle pregnancy group,AP,CC,C,and MSA in late pregnancy group increased significantly(P<0.05).The consistency of transabdominal three-dimensional ultrasound VCI-C and gold standard was high(Kappa=0.526,P<0.05).Gestation-al age was positively correlated with AP(r=0.562,P<0.05),CC(r=0.441,P<0.05),C(r=0.547,P<0.05),and MSA(r=0.582,P<0.05).Conclusion The diagnosis of fetal posterior fossa cystic abnormalities by transabdominal three-dimensional ultrasound VCI-C is consistent with the existing gold standard results,and gestational age is significantly correlated with the diagnostic indicators of transabdominal three-dimensional ultrasound VCI-C.
