COPD患者支气管肺泡灌洗液中BLMPs数目和白细胞介素水平的变化及其与病情进展的相关性

摘要 目的 检测慢性阻塞性肺疾病(COPD)患者支气管肺泡灌洗液(BALF)中B淋巴细胞微粒(BLMPs)数目和白细胞介素(IL)水平,并分析BLMPs数目、IL水平与COPD病情进展的相关性,为临床早期诊断提供理论指导。方法 回顾性分析2020年9月至2023年5月于郑州颐和医院就诊的264例COPD患者的临床资料,将其分为稳定期组138例和急性进展组126例。采集两组患者的支气管肺泡灌洗液,检测并比较支气管肺泡灌洗液中BLMPs数目及IL-6、IL-8、IL-38水平;采用线性回归分析影响COPD患者BLMPs数目的因素,采用Pearson相关性分析BLMPs数目、IL-6、IL-8、IL-38水平与COPD病情进展的相关性。结果 急性进展组患者的BLMPs数目为(45.37±5.19)个/100μL,明显低于稳定期组的(113.02±9.41)个/100μL,差异具有统计学意义(P<0.05);线性回归分析结果提示年龄与体表面积均不是COPD患者BLMPs数目的影响因素(P>0.05);急性进展组患者的IL-6、IL-8、IL-38水平分别为(25.36±3.27) ng/L、(17.49±3.35) ng/L、(58.28±13.75) ng/L,明显高于稳定期组的(18.81±3.42) ng/L、(13.84±2.46) ng/L、(52.17±14.11) ng/L,差异均有统计学意义(P<0.05);Pearson相关性分析结果显示,支气管肺泡灌洗液中BLMPs数目与COPD病情进展呈负相关(r=-0.747,P<0.05),IL-6 (r=0.703,P<0.05)、IL-8 (r=0.590,P<0.05)、IL-38 (r=0.283,P<0.05)水平与COPD病情进展呈正相关。结论 BALF中BLMPs数目、IL-6、IL-8、IL-38水平与COPD病情进展密切相关,可作为早期诊断与评估病情的有效指标。 Objective To detect the number of B lymphocyte particles(BLMPs)and the level of interleukins(IL)in bronchoalveolar lavage fluid(BALF)of chronic obstructive pulmonary disease(COPD),and analyze the correlation be-tween the number of BLMPs,IL level and the progression of COPD disease,so as to provide theoretical guidance for early clinical diagnosis.Methods The clinical data of 264 patients with COPD who were admitted to Zhengzhou Yihe Hospital from September 2020 to May 2023 were retrospectively analyzed,and the patients were divided into a stable group(138 cases)and an acute progression group(126 cases).The bronchoalveolar lavage fluid of the two groups was collected,and the number of BLMPs and the levels of IL-6,IL-8 and IL-38 in the bronchoalveolar lavage fluid was detected and compared.Linear re-gression analysis was used to analyze the factors affecting the number of BLMPs in COPD patients.Pearson correlation was used to analyze the correlation between the number of BLMPs,IL-6,IL-8,IL-38 levels and disease progression of COPD.Results The number of BLMPs in the acute progression group was(45.37±5.19)per 100μL,which was significantly lower than(113.02±9.41)per 100μL in the stable group(P<0.05).Linear regression analysis suggested that age and body surface area were not influencing factors on the number of BLMPs in COPD patients(P>0.05).The levels of IL-6,IL-8,and IL-38 in the acute progression group were(25.36±3.27)ng/L,(17.49±3.35)ng/L,and(58.28±13.75)ng/L,respectively,significantly higher than(18.81±3.42)ng/L,(13.84±2.46)ng/L,and(52.17±14.11)ng/L in the stable group(P<0.05).Pearson correlation analysis showed that the number of BLMPs in bronchoalveolar lavage fluid was negative-ly associated with disease progression of COPD(r=-0.747,P<0.05),while IL-6(r=0.703,P<0.05),IL-8(r=0.590,P<0.05),IL-38(r=0.283,P<0.05)were positively associated with disease progression of COPD.Conclusion The number of BLMPs and levels of IL-6,IL-8,and IL-38 in BALF are closely related to the disease progression of COPD,which can be used as effective indicators for early diagnosis and evaluation of the disease.
