“从阳引阴”理论选穴针刺对慢性盆腔痛患者的临床价值

Clinical value of acupoint selection acupuncture based on the theory of‘Guiding Yin from Yang'in patients with chronic pelvic pain

目的:探讨“从阳引阴”理论选穴针刺对慢性盆腔痛(CPP)患者的临床价值。方法:选择2021年11月—2023年12月收治的慢性盆腔痛患者106例,根据随机字母表法分为观察组和对照组,每组53例。对照组给予布洛芬治疗,观察组采用“从阳引阴”理论选穴进行针刺治疗,选穴为八髎、肾俞、秩边、命门、三阴交。两组治疗周期均为3个月。采用视觉模拟评分法(VAS)和盆腔体征量表(Mc Cormack)对患者的主观和客观疼痛进行评估;比较两组临床总有效率、盆底功能、生活质量评分以及不良反应发生情况。结果:两组CPP患者治疗后VAS和Mc Cormack量表评分较治疗前显著下降,且观察组低于对照组(P<0.05);观察组临床总有效率明显高于对照组(P<0.05)。治疗后观察组生活质量评分中的疼痛、控制无力感、情感健康评分以及总分低于对照组,差异均有统计学意义(P<0.05)。两组盆底功能障碍发生率和不良反应发生率比较,差异均无统计学意义(P>0.05)。结论:“从阳引阴”理论选穴针刺疗效显著,可明显改善患者疼痛症状,提高慢性盆腔痛患者的生活质量,安全性高。

Objective:To explore the clinical value of acupoint acupuncture based on the theory of"Guiding Yin from Yang"in patients with chronic pelvic pain(CPP).Methods:A total of 106 patients with chronic pelvic pain in hospital from November 2021 to December 2023 were randomly divided into treatment group and control group,with 53 patients in each group.The control group was treated with conventional ibuprofen,and the observation group was treated with acupuncture according to the theory of`inducing Yin from Yang',including Baluo,Shenshu,Zhibian,Mingmen and Sanyinjiao.The treatment cycle of the two groups was 3 months.The visual analogue scale(VAS)and the pelvic examination scale(Mc Cormack)were used to evaluate the subjective and objective pain of the patients respectively.The clinical total effective rate,quality of life score and pelvic floor function were compared between the two groups after treatment.Results:The vas and MC Cormack scale scores of CPP patients in the two groups after treatment were significantly decreased compared with those before treatment.The vas and MC Cormack scale scores of the observation group after treatment were significantly lower than those of the control group(P<0.05).In terms of clinical total effective rate,the observation group was significantly higher than that of the control group(P<0.05).The pain,control powerlessness,emotional health score and total score of the observation group after treatment were significantly improved compared with those of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of pelvic floor dysfunction between the two groups(P>0.05).Conclusion:Acupuncture Based on the theory of`guiding Yin from Yang'can effectively improve the pain symptoms of patients,significantly improve the quality of life,with significant curative effect,high safety.
