生物波早期综合康复治疗重型颅脑损伤昏迷临床观察

Clinical Observation on Early Comprehensive Rehabilitation Treatment with Bio-Waves in Coma Patients with Heavy Craniocerebral Injury

摘要 目的探讨生物波早期综合康复治疗重型颅脑损伤昏迷的临床效果。方法选取医院2022年1月至2023年6月收治的重型颅脑损伤昏迷患者120例,按随机抽签法分为对照组和观察组,各60例。对照组患者予常规药物治疗,观察组患者予生物波早期综合康复治疗。对比两组患者总动态脑电图(AEEG)异常情况,治疗前后脑血流动力学指标及格拉斯哥昏迷评分。结果观察组患者AEEG异常率为53.33%,显著低于对照组的81.67%(P<0.05)。两组患者治疗后的动脉收缩期最大流速(Vs)、舒张期最大流速(Vd)、平均流速(Vm)水平均显著升高,搏动指数(PI)、阻力指数(RI)均显著缩小,GOS评分均显著升高(P<0.05);且观察组上述指标均显著优于对照组(P<0.05)。结论生物波早期综合康复治疗重型颅脑损伤昏迷,能调节患者的脑血流动力学,缓解脑血管痉挛与昏迷情况。 Objective To investigate the clinical effect of early comprehensive rehabilitation treatment with bio-wave in coma patients with heavy craniocerebral injury.Methods A total of 120 coma patients with severe craniocerebral injury admitted to the hospital from January 2022 to June 2023 were selected and divided into control group and observation group according to the method of random drawing,with 60 cases in each group.The patients in the control group were treated with conventional drugs,and the patients in the observation group were treated with bio-wave early comprehensive rehabilitation.The total dynamic electroencephalogram(AEEG)abnormalities,cerebral hemodynamic indexes before and after treatment,and Glasgow coma score were compared between the two groups.Results The AEEG abnormality rate in the observation group was 53.33%,which was significantly lower than 81.67%in the control group(P<0.05).The maximum systolic velocity(Vs),maximum diastolic flow velocity(Vd),and mean flow velocity(Vm)levels of the arteries in the two groups were significantly higher after treatment,the pulsatility index(PI)and resistance index(RI)were significantly lower,and the GOS score was significantly higher(P<0.05);and the above indexes of the observation group were significantly better than those in the control group(P<0.05).Conclusion Bio-wave early comprehensive rehabilitation treatment in coma patients with heavy craniocerebral injury can regulate the cerebral hemodynamics and relieve cerebral vascular spasm and coma.
