根管治疗后短冠磨牙应用带桩髓腔固位冠修复的疗效评估

Evaluation of the efficacy of post-core-retained crown in the restoration of short-crowned molars after root canal treatment

目的:评估根管治疗后短冠磨牙应用带桩髓腔固位冠修复的疗效。方法:选取2018年1月—2021年12月于兰州市口腔医院完善根管治疗的32例短冠磨牙患者作为研究对象,根据每颗牙齿的剩余牙体组织量和根管分布情况选择2~3个根管,去除根管上段牙胶,进行少量桩道预备或不进行桩道预备,使髓腔和根管深度总和为4~5 mm,桩道分布于患牙颊舌(腭)侧和近远中,修复体设计并制作完成,口内试戴后黏接。修复完成后进行临床复查2年,根据世界牙科联盟修复体评价标准对修复体质量进行评分,分析带桩髓腔固位冠用于短冠磨牙修复的成功率。结果:32例患者中,成功31例,失败1例,成功率为96.9%。结论:带桩髓腔固位冠可以应用于根管治疗后髓腔固位深度不足3 mm的短冠磨牙。

Objective To evaluate the effect of post-core-retained crown in the restoration of short-crowned molars after root canal.Methods A total of 32 cases of short crown molar patients who had received perfect root canal therapy in Lanzhou Stomatology Hospital from January 2018 to December 2021 were selected as research subjects.2-3 root canals were chosen based on the remaining tooth structure volume and root canal distribution of each tooth,with the upper part of the root canal gutta-percha removed.A small amount of dowel preparation or no dowel preparation was performed to ensure that the total depth of the pulp chamber and root canal was within 4-5 mm,with the dowel distributed on the buccal(palatal)side and mesial/distal of the affected tooth.The restoration was designed and manufactured,and then tried in place intraorally before being cemented.Clinical follow-up was conducted 2 years after the restoration was completed.The quality of the restoration was evaluated by scoring according to the Fédération Dentalire Internationale restoration evaluation criteria,and the success rate of the post-core-retained crownused for the restoration of short-crowned molars was statistically analyzed.Results Among the 32 cases,31 were successful and 1 was failed,with a success rate of 96.9%.Conclusions The post-core-retained crown can be applied to the restoration of short-crowned molars with insufficient pulp chamber retention depth of less than 3 mm after root canal treatment.
