显微镜辅助下血运重建术对年轻恒牙牙髓坏死患者牙根发育情况、疼痛程度的影响

Effect of Microscope-assisted Dental Pulp Revascularization on Root Development and Pain Degree in Patients with Pulp Necrosis of Young Permanent Teeth

目的:探讨年轻恒牙牙髓坏死患者采取显微镜辅助下血运重建术(DPR)的效果。方法:随机将2019年1月—2023年1月龙岩人民医院收治的年轻恒牙牙髓坏死患者80例(患牙80颗)分为两组,各40例,观察组采取DPR,对照组采取根尖诱导成形术(Ap),两组患者随访12个月。比较两组的治疗效果、疼痛程度、牙根发育状况、牙骨样组织沉积率及并发症发生情况。结果:观察组治疗总有效率比对照组高(P<0.05);两组术后视觉模拟评分法(VAS)评分均较术前降低,且观察组比对照组低(P<0.05);两组术后12个月的牙根管壁厚度、冠根比、牙根长度均较术前升高,且观察组均比对照组高(P<0.05);观察组术后3、6、12个月的牙骨样组织沉积率均比对照组高(P<0.05);观察组并发症发生率比对照组低(P<0.05)。结论:年轻恒牙牙髓坏死患者采取DPR效果满意,可减轻患者疼痛程度,促进牙根发育,且有较高的安全性。

Objective:To explore the effect of microscope-assisted dental pulp revascularization(DPR)in patients with pulp necrosis of young permanent teeth.Method:A total of 80 patients(80 affected teeth)with pulp necrosis of young permanent teeth admitted to Longyan People's Hospital from January 2019 to January 2023 were randomly divided into two groups,each with 40 cases.The observation group received DPR and the control group underwent apexification(Ap).Patients in both groups were followed up for 12 months.The therapeutic effect,pain degree,root development,cementoid tissue deposition rate and complications were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group(P<0.05).The visual analogue scale(VAS)scores of both groups after surgery decreased compared with those before surgery,and that of the observation group was lower than that of the control group(P<0.05).The root canal wall thickness,crown root ratio and root length of both groups after surgery for 12 months increased compared with those before surgery,and those of the observation group were higher than those of the control group(P<0.05).The cementoid tissue deposition rates of the observation group in 3,6 and 12 months after surgery were higher than those of the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:The effect of DPR in patients with pulp necrosis of young permanent teeth is satisfactory.It can reduce the patients'pain degree and promote tooth root development,which has high safety.
