气道廓清技术联合体外震动排痰仪在脑卒中后相关性肺炎中的应用效果

Application Effect of Airway Clearance Technology Combined with Extracorporeal Shock Sputum Extraction Apparatus in Post-stroke Associated Pneumonia

摘要 目的:探析气道廓清技术联合体外震动排痰仪应用于脑卒中后相关性肺炎患者中的效果。方法:选择2020年1月—2022年12月蚌埠医科大学第一附属医院神经内科重症监护室收治的90例脑卒中后相关性肺炎患者,以随机数字表法分成对照组和观察组,各45例。两组均接受常规护理干预,在此基础上,对照组给予气道廓清技术,观察组给予气道廓清技术联合体外震动排痰仪干预。比较两组的咳痰评分、呼吸困难程度[呼吸困难指数量表(mMRC)]、肺部感染情况[临床肺部感染评分(CPIS)]及干预满意度。结果:干预7、14 d后,观察组咳痰评分和mMRC评分均低于对照组,差异均有统计学意义(P<0.05)。干预7 d后,观察组CPIS各方面评分均低于对照组,差异均有统计学意义(P<0.05)。观察组总满意度高于对照组,差异有统计学意义(P<0.05)。结论:体外震动排痰仪联合气道廓清技术应用于脑卒中后相关性肺炎患者中效果显著,能够促进排痰,减轻肺部感染,改善呼吸困难症状,干预满意度高。 Objective:To explore the effect of airway clearance technology combined with extracorporeal shock sputum extraction apparatus in patients with post-stroke associated pneumonia.Method:A total of 90 patients with post-stroke associated pneumonia admitted to the Intensive Care Unit of the Department of Neurology,the First Affiliated Hospital of Bengbu Medical University from January 2020 to December 2022 were selected and divided into control group and observation group by random number table method,with 45 cases in each group.Both groups received routine nursing intervention,on the basis of which,the control group was given airway clearance technology,and the observation group was given airway clearance technology combined with extracorporeal shock sputum extraction apparatus intervention.The sputum score,dyspnea degree[modified medical research council(mMRC)],and pulmonary infection[clinical pulmonary infection score(CPIS)]and intervention satisfaction of the two groups were compared.Result:After 7 and 14 d of intervention,sputum scores and mMRC scores in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After 7 d of intervention,CPIS scores in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The total satisfaction of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion:The application of extracorporeal shock sputum extraction apparatus combined with airway clearance technology in patients with post-stroke associated pneumonia has significant effects,which can promote expectoration,reduce pulmonary infection,improve dyspnea symptoms,and increase intervention satisfaction.
