咳喘合剂对腺病毒肺炎患儿肺通气功能、炎症因子和免疫球蛋白的影响

Effect of Kechuan Admixture on Pulmonary Ventilation Function,Inflammatory Factors,and Immunoglobulins in Children with Adenovirus Pneumonia

摘要 目的:探索咳喘合剂对腺病毒肺炎患儿的肺功能、炎症反应及免疫球蛋白水平的调节效果。方法:选择2022年2月—2024年1月常州市武进中医医院收治的102例腺病毒肺炎患儿作为研究对象,根据随机数字表法将其分为对照组(n=51)和试验组(n=51)。对照组给予常规西药治疗,试验组则在对照组基础上增加咳喘合剂辅助治疗。比较两组临床指标、肺通气功能、炎症因子、免疫球蛋白水平及不良反应发生率。结果:试验组止咳时间、退热时间、湿啰音消失时间均早于对照组,住院时间短于对照组,差异均有统计学意义(P<0.05)。治疗后,试验组吸气时间(TI)、呼气时间(TE)、吸呼比(TI/TE)、达峰时间比(TPTEF/TE)及达峰容积比(VPEF/VE)均高于对照组,差异均有统计学意义(P<0.05)。治疗后,试验组肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)、Clara细胞分泌蛋白(CCSP)水平均低于对照组,而免疫球蛋白G(IgG)、免疫球蛋白A(IgA)及免疫球蛋白M(IgM)水平均高于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:咳喘合剂能显著改善腺病毒肺炎患儿的肺通气功能,减轻炎症反应及调节免疫球蛋白水平,且安全性良好。 Objective:To explore the regulation effect of Kechuan Admixture on pulmonary function,inflammatory response and immunoglobulin level in children with adenovirus pneumonia.Method:A total of 102 children with adenovirus pneumonia admitted to Changzhou Wujin Hospital of Traditional Chinese Medicine from February 2022 to January 2024 were selected as the study objects,and they were divided into control group(n=51)and experimental group(n=51)according to random number table method.The control group was treated with conventional western medicine,and the experimental group was treated with Kechuan Admixture.The clinical indexes,pulmonary ventilation function,inflammatory factors,immunoglobulin levels and the incidence of adverse reactions were compared between the two groups.Result:The cough relief time,fever remission time and moist rales disappearance time of experimental group were earlier than those of control group,and the hospital stay was shorter than that of control group,the differences were statistically significant(P<0.05).After treatment,inspiratory time(TI),expiratory time(TE),inhale-to-breathe ratio(TI/TE),ratio of time to peak tidal expiratory flow to total expiratory time(TPTEF/TE)and ratio of volume at peak tidal expiratory flow to expiratory tidal volume(VPEF/VE)in experimental group were higher than those in control group,the differences were statistically significant(P<0.05).After treatment,levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-2(IL-2),and Clara cell secretory protein(CCSP)in the experimental group were lower than those in the control group,the levels of immunoglobulin G(IgG),immunoglobulin A(IgA),and immunoglobulin M(IgM)were higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The Kechuan Admixture can significantly improve the pulmonary ventilation function,reduce inflammation and adjust the level of immunoglobulin in children with adenovirus pneumonia with good safety.
