腹腔镜下输卵管切开取胚术在输卵管妊娠患者中的临床效果观察

Observation of Clinical Effect of Laparoscopic Tubal Incision Embryo Extraction in Patients with Tubal Pregnancy

目的:观察腹腔镜下输卵管切开取胚术在输卵管妊娠患者中的临床效果。方法:选取徐州和平妇产医院2022年2月—2023年2月收治的79例输卵管妊娠患者。将患者按随机数字表法分为对照组39例与观察组40例,对照组进行腹腔镜下输卵管开窗取胚术,观察组进行腹腔镜下输卵管切开取胚术。对比两组手术相关指标(手术时间、术中出血量、住院时间)、β-人绒毛膜促性腺激素(β-hCG)水平、输卵管通畅率及妊娠情况。结果:与对照组对比,观察组手术时间更长(P<0.05);两组术中出血量、住院时间对比,差异均无统计学意义(P>0.05)。两组术前与术后3 d的β-hCG水平对比,差异均无统计学意义(P>0.05)。观察组宫内妊娠率、输卵管通畅率均高于对照组(P<0.05),两组异位妊娠复发率对比,差异无统计学意义(P>0.05)。结论:对输卵管妊娠患者进行手术治疗过程中,患者接受腹腔镜下输卵管切开取胚术虽然手术时间长,但能有效提升输卵管通畅率,促进宫内妊娠率的提升,临床的实际应用可根据患者具体需求选择合适手术方式。

Objective:To observe the clinical effect of laparoscopic tubal incision embryo extraction in patients with tubal pregnancy.Method:A total of 79 patients with tubal pregnancy admitted to Xuzhou Peace Maternity Hospital from February 2022 to February 2023 were selected.The patients were divided into control group(39 cases)and observation group(40 cases)according to random number table method.The control group received laparoscopic tubal fenestration embryo extraction,while the observation group received laparoscopic tubal incision embryo extraction.The operation related indexes(operation time,intraoperative blood loss,length of hospital stay),β-human chorionic gonadotropin(β-hCG)level,tubal patency rate and pregnancy status were compared between the two groups.Result:Compared with control group,the operation time of observation group was longer(P<0.05).There were no significant differences in intraoperative blood loss and hospital stay between the two groups(P>0.05).There were no significant differences inβ-hCG level between the two groups before and 3 days after surgery(P>0.05).The intrauterine pregnancy rate and fallopian tube patency rate of the observation group were higher than those of the control group(P<0.05).There was no significant difference in the recurrence rate of ectopic pregnancy between the two groups(P>0.05).Conclusion:In the process of surgical treatment for patients with tubal pregnancy,although the operation time of patients undergoing laparoscopic tubal incision embryo removal is long,it can effectively improve the patency rate of fallopian tube and promote the improvement of intrauterine pregnancy rate.The practical clinical application can select the appropriate surgical method according to the specific needs of patients.
