摘要
目的:探讨MRI联合胱抑素C(CysC)、脑钠肽(BNP)对重度急性缺血性脑卒中(AIS)的诊断价值。方法:选取2022年4月—2024年5月于宜春市人民医院就诊的60例AIS患者作为研究对象,根据AIS严重程度分为重度AIS组、轻中度AIS组,比较两组的MRI参数[脑血流量(CBF)、表观弥散系数(ADC)]、血清CysC、BNP指标。应用受试者操作特征(ROC)曲线分析MRI联合血清CysC、BNP对重度AIS的诊断价值。结果:重度AIS组CBF、ADC值均低于轻中度AIS组,而血清CysC、BNP水平高于轻中度AIS组(P<0.05)。MRI参数、CysC、BNP联合诊断重度AIS的敏感度高于CBF、ADC、CysC、BNP单项评估(χ^(2)=5.700、7.134、8.686、10.364,P=0.017、0.008、0.003、0.001),联合诊断的AUC高于CBF、ADC、CysC、BNP单项评估(Z=3.509、3.479、3.003、2.687,P=0.000、0.000、0.003、0.007)。结论:MRI联合血清CysC、BNP诊断重度AIS优于单项诊断,可有效减少误诊、漏诊。
Objective:To investigate the diagnostic value of MRI combined with cystatin C(CysC)and brain natriuretic peptide(BNP)in severe acute ischemic stroke(AIS).Method:A total of 60 patients with AIS who were treated in Yichun People's Hospital from April 2022 to May 2024 were selected as the study subjects,and divided into severe AIS group and mild-moderate AIS group according to the severity of AIS.MRI parameters[cerebral blood flow(CBF),apparent diffusion coefficient(ADC)],serum CysC and BNP indexes of the two groups were compared.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of MRI combined with serum CysC and BNP in severe AIS.Result:The values of CBF and ADC in severe AIS group were lower than those in mild-moderate AIS group,while the levels of CysC and BNP in serum were higher than those in mildmoderate AIS group(P<0.05).The sensitivity of MRI parameters,CysC and BNP combined diagnosis in severe AIS was higher than those of CBF,ADC,CysC and BNP(χ^(2)=5.700,7.134,8.686,10.364,P=0.017,0.008,0.003,0.001).The AUC of combined diagnosis was higher than those of CBF,ADC,CysC and BNP(Z=3.509,3.479,3.003,2.687,P=0.000,0.000,0.003,0.007).Conclusion:MRI combined with serum CysC and BNP in the diagnosis of severe AIS is superior to single diagnosis,which can effectively reduce misdiagnosis and missed diagnosis.
