摘要
目的:探讨组织蛋白酶D(Cath-D)和微小RNA-21(miR-21)表达水平联合双能量CT能谱数据在肾癌和乏脂肪肾错构瘤鉴别诊断中的应用价值。方法:本研究选取抚州市第一人民医院2020年1月—2022年12月收治的肾错构瘤患者100例作为研究对象,根据术后病理结果将患者分为肾癌组(n=47)和肾错构瘤组(n=53)。所有患者均进行双能量CT扫描和Cath-D、miR-21表达水平测定。使用独立样本t检验和χ^(2)检验分析两组间的能谱曲线斜率、配对物质浓度、Cath-D、miR-21表达水平差异。进行影响肾癌的多因素分析,并采用ROC曲线分析Cath-D、miR-21表达水平、能谱曲线斜率在肾癌诊断中的效能。结果:肾错构瘤组的Cath-D、miR-21表达水平均显著低于肾癌组(P<0.05)。肾癌组能谱曲线斜率显著低于肾错构瘤组(P<0.05)。ROC曲线分析结果显示,Cath-D、miR-21表达水平联合能谱曲线斜率的AUC为0.953,高于单一指标。结论:Cath-D和miR-21表达水平联合双能量CT能谱数据对于肾癌和乏脂肪肾错构瘤的鉴别诊断具有重要的临床价值,可以为临床提供更准确的诊断依据,有助于指导患者的治疗方案选择和预后评估。
Objective:To explore the application value of combining cathepsin D(Cath-D)and microRNA-21(miR-21)expression levels with dual-energy CT spectral data in differentiating renal cancer from renal hamartoma without visible fat.Method:A total of 100 patients with renal hamartoma treated at the First People's Hospital of Fuzhou from January 2020 to December 2022 were selected as study subjects.Based on postoperative pathological results,patients were divided into renal cancer group(n=47)and renal hamartoma group(n=53).All patients underwent dual-energy CT scans and assessments of Cath-D and miR-21 expression levels.The differences in spectral curve slope,paired material concentration,and Cath-D and miR-21 expression levels between the two groups were analyzed using independent sample t-tests andχ^(2) tests.Multifactorial analysis of factors affecting kidney cancer was perform and ROC curve analysis was used to evaluate the diagnostic effectiveness of Cath-D,miR-21 expression levels,and the slope of the energy spectrum curve in kidney cancer diagnosis.Result:The expression levels of Cath-D and miR-21 in the renal hamartoma group were significantly lower than those in the renal cancer group(P<0.05).The slope of the spectral curve for renal cancer group was significantly lower than that of the renal hamartoma group(P<0.05).The AUC of combining Cath-D and miR-21 expression levels with the spectral curve slope was 0.953,which was higher than that of any single indicator.Conclusion:The combination of Cath-D and miR-21 expression levels with dual-energy CT spectral data plays a significant clinical role in differentiating renal cancer from renal hamartoma,providing a more accurate diagnostic basis for clinical practice and aiding in the selection of treatment plans and prognosis assessment for patients.
