摘要
目的:探究颈总动脉收缩期最大流速(PSV)联合白蛋白/纤维蛋白原比值(AFR)评估颈动脉狭窄严重程度的价值。方法:回顾性分析2021年6月—2023年6月宜春市人民医院收治的106例颈动脉狭窄患者的病历资料,将其纳入颈动脉狭窄组,另选取本院同期收治的84例非颈动脉狭窄患者纳入非颈动脉狭窄组;根据数字减影血管造影检查结果,将颈动脉狭窄组中重度颈动脉狭窄患者纳入重度狭窄组(n=32),轻中度狭窄患者纳入轻中度狭窄组(n=74)。比较颈动脉狭窄组和非颈动脉狭窄组、重度狭窄组和轻中度狭窄组的颈总动脉PSV、AFR,分析颈总动脉PSV、AFR评估颈动脉狭窄严重程度的价值。结果:颈动脉狭窄组的颈总动脉PSV、AFR水平均高于非颈动脉狭窄组,差异均有统计学意义(P<0.05);重度狭窄组的颈总动脉PSV、AFR水平均高于轻中度狭窄组,差异均有统计学意义(P<0.05)。受试者操作特征(ROC)曲线显示,颈总动脉PSV、AFR联合预测颈动脉狭窄严重程度的曲线下面积(AUC)为0.888,高于颈总动脉PSV、AFR的0.835、0.790。结论:相比于颈总动脉PSV、AFR单独评估,颈总动脉PSV联合AFR评估颈动脉狭窄严重程度的价值更高。
Objective:To explore the value of common carotid artery peak systolic velocity(PSV)combined with albumin/fibrinogen ratio(AFR)in assessing the severity of carotid artery stenosis.Method:The medical records of 106 patients with carotid artery stenosis admitted to Yichun People's Hospital from June 2021 to June 2023 were retrospectively analyzed and included in the carotid artery stenosis group.Another 84 patients with non-carotid artery stenosis admitted to our hospital during the same period were included in the non-carotid artery stenosis group.According to the results of digital subtraction angiography,patients with severe carotid stenosis in the carotid artery stenosis group were included in the severe stenosis group(n=32),and those with mild to moderate stenosis were included in the mild to moderate stenosis group(n=74).Common carotid artery PSV and AFR of carotid artery stenosis group were compared with those of non-carotid artery stenosis group,severe stenosis group and mild to moderate stenosis group,and the value of common carotid artery PSV and AFR in evaluating the severity of carotid artery stenosis was analyzed.Result:The levels of common carotid artery PSV and AFR in carotid artery stenosis group were higher than those in non-carotid artery stenosis group,the differences were statistically significant(P<0.05).The common carotid artery PSV and AFR levels in the severe stenosis group were higher than those in the mild to moderate stenosis group,the differences were statistically significant(P<0.05).Receiver operating characteristic(ROC)curve showed that the area under curve(AUC)of common carotid artery PSV combined with AFR in predicting severity of carotid artery stenosis was 0.888,which was higher than 0.835 and 0.790 of common carotid artery PSV and AFR.Conclusion:The common carotid artery PSV combined with AFR is of higher value in assessing the severity of carotid artery stenosis than the assessment of common carotid artery PSV,AFR alone.
