脱细胞异体真皮联合自体刃厚皮片移植修复头面部深度烧伤创面的效果

Effect of Acellular Allogenic Dermis Combined with Autologous Blade Thick Skin Grafts in Repairing Deep Burn Wounds of Head and Face

目的:探究脱细胞异体真皮联合自体刃厚皮片移植在头面部深度烧伤创面中的修复效果。方法:选取2021年6月—2023年6月贵州省人民医院收治的64例头面部深度烧伤患者作为研究对象,利用双盲法将其分为对照组和联合组,每组各32例。对照组采用自体刃厚皮片移植修复,联合组在此基础上联合脱细胞异体真皮治疗。对比两组移植皮片成活率,观察创面愈合指标,统计并发症发生率,以温哥华瘢痕量表(VSS)评估两组术后瘢痕增生情况。结果:联合组移植皮片成活率与对照组比较,差异无统计学意义(P>0.05);联合组创面干燥时间、创面愈合时间短于对照组,差异有统计学意义(P<0.05);联合组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);术后3个月及6个月联合组VSS各项评分及总得分低于对照组,差异有统计学意义(P<0.05)。结论:予以头面部深度烧伤患者脱细胞异体真皮联合自体刃厚皮片移植修复能有效确保移植皮片成活,加速创面愈合,降低相关并发症发生风险,并减少瘢痕增生,修复效果较好。

Objective:To investigate the repair effect of acellular allograft dermis combined with autologous blade thick skin in deep burn wounds of head and face.Method:64 patients with deep head and face burn admitted to Guizhou Provincial People's Hospital from June 2021 to June 2023 were selected as the study objects.They were divided into control group and combination group by doubleblind method,with 32 cases in each group.The control group was repaired by autologous thick blade skin graft,and the combination group was treated with acellular allogeneic dermis on this basis.The survival rate of the two groups,the wound healing index and the incidence of complications were compared,and the scar hyperplasia of the two groups was evaluated by the Vancouver Scar Scale(VSS).Result:There was no significant difference in the survival rate of skin between the combined group and the control group(P>0.05).The wound drying time and wound healing time in combination group were shorter than those in control group(P<0.05).The incidence of postoperative complications in combination group was lower than that in control group(P<0.05).All VSS scores and total scores of the combined group were lower than those of the control group 3 months and 6 months after operation(P<0.05).Conclusion:Acellular allograft dermis combined with autologous blade thick skin can effectively ensure skin survival,accelerate wound healing,reduce the risk of related complications,and reduce scar hyperplasia in patients with deep head and face burns,and the repair effect is better.
