痛风合剂3号方治疗慢性尿酸性肾病脾肾亏虚证的临床效果

Clinical Effect of Gout Mixture No.3 in the Treatment of Chronic Uric Acid Nephropathy with Spleen and Kidney Deficiency Syndrome

目的:探讨慢性尿酸性肾病(CUAN)脾肾亏虚证患者采用痛风合剂3号方治疗的临床效果。方法:选取2017年1月—2022年12月泉州市中医院收治的60例CUAN脾肾亏虚证患者作为研究对象,根据随机数表法分为研究组和对照组,每组各30例。对照组给予常规西药治疗,研究组给予痛风合剂3号方治疗。比较两组临床疗效、中医症候积分、肾功能、炎症反应及不良反应情况。结果:研究组治疗总有效率较对照组高,差异有统计学意义(P<0.05);治疗后,两组中医症候积分均下降,研究组较对照组更低,差异有统计学意义(P<0.05);治疗后,两组尿素氮(BUN)、24 h尿蛋白定量(24 hUpro)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)均下降,肾小球滤过率(eGFR)升高,研究组BUN、24hUpro、NGAL较对照组更低,eGFR更高,差异有统计学意义(P<0.05);治疗后,两组白细胞介素-6(IL-6)、C反应蛋白(CRP)水平下降,研究组较对照组更低,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:CUAN脾肾亏虚证患者采用痛风合剂3号方治疗,能够提高临床疗效,缓解中医症候,改善肾功能的同时降低炎症反应,且不会增加不良反应。

Objective:To explore the clinical effect of using gout mixture No.3 in the treatment of chronic uric acid nephropathy(CUAN)patients with spleen and kidney deficiency syndrome.Method:A total of 60 patients with CUAN spleen-kidney deficiency syndrome treated in Quanzhou Hospital of Traditional Chinese Medicine from January 2017 to December 2022 were selected as study objects and divided into study group and control group according to random number table method,with 30 cases in each group.The control group was treated with conventional western medicine,while the study group was treated with gout mixture No.3.The clinical efficacy,TCM symptom score,renal function,inflammatory reaction and adverse reaction were compared between the two groups.Result:The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of TCM symptoms of both groups decreased,and the study group was lower than the control group,and the difference was statistically significant(P<0.05).After treatment,urea nitrogen(BUN),24 h urinary protein quantity(24 hUpro)and neutrophil gelatinase-associated lipid carrier protein(NGAL)decreased in both groups,and glomerular filtration rate(eGFR)increased.BUN,24 hUpro and NGAL in the study group were lower than those in the control group,but eGFR was higher.The difference was statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6)and C-reactive protein(CRP)in the two groups decreased,and the levels in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in comparison of the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Treating CUAN patients with spleen-kidney deficiency syndrome with Gout Mixture No.3 can improve clinical efficacy,relieve TCM symptoms,improve renal function and reduce inflammation without increasing adverse reactions.
