达格列净联合利拉鲁肽对肥胖2型糖尿病患者糖脂代谢指标及胰岛功能的影响

Effects of Dapagliflozin Combined with Liraglutide on Glucose and Lipid Metabolism Indexes and Islet Function in Obese Patients with Type 2 Diabetes Mellitus

目的:探讨达格列净联合利拉鲁肽对肥胖2型糖尿病(T2DM)患者糖脂代谢指标及胰岛功能的影响。方法:选取2019年6月—2024年4月蚌埠医科大学第二附属医院收治的102例肥胖T2DM患者,随机分为对照组(n=50)、联合组(n=52)。对照组采用利拉鲁肽治疗,联合组采用利拉鲁肽联合达格列净治疗。比较两组体重指数(BMI)、腰围、糖脂代谢指标、胰岛功能及不良反应发生率。结果:治疗12周后,两组BMI、腰围、总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FPG)、餐后2 h血糖(2 h PG)和空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平均低于治疗前,且联合组上述各项指标均低于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:达格列净联合利拉鲁肽治疗肥胖T2DM具有良好的减重效果,能有效调节机体糖脂代谢,改善胰岛功能,且安全性良好。

Objective:To investigate the effects of Dapagliflozin combined with Liraglutide on glucose and lipid metabolism and islet function in obese type 2 diabetes mellitus(T2DM)patients.Method:A total of 102 obese T2DM patients admitted to Second Affiliated Hospital of Bengbu Medical University from June 2019 to April 2024 were selected and randomly divided into control group(n=50)and combination group(n=52).The control group was treated with Liraglutide,and the combination group was treated with Liraglutide combined with Dapagliflozin.Body mass index(BMI),waist circumference,glucose and lipid metabolism indexes,islet function and incidence of adverse reactions were compared between the two groups.Result:After 12 weeks of treatment,BMI,waist circumference,total cholesterol(TC),triglyceride(TG),fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 h PG),fasting insulin(FINS)and homeostasis model assessment for insulin resistance(HOMA-IR)levels in both groups were lower than those before treatment,and the above indexes in the combined group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Dapagliflozin combined with Liraglutide has a good effect on weight loss in obese T2DM,can effectively regulate the body's glucose and lipid metabolism,improve the function of pancreatic islets,and has good safety.
