摘要
目的探讨“互联网+护理”背景下分阶段回授法在食管癌围手术期营养管理中的应用效果。方法选取苏北人民医院胸外科2023年9月至2024年2月行胸腔镜食管癌根治术的患者为研究对象,其中将2023年9月至11月手术的40例患者纳入对照组,将2023年12月至2024年2月手术的40例患者纳入试验组。对照组实施常规营养管理措施,试验组在常规营养管理的基础上实施“互联网+护理”背景下分阶段回授法。比较两组患者入院时、出院时、出院1个月、出院2个月、出院3个月的营养状态,包括患者主观全面评估(PG-SGA)、血清白蛋白(ALb)、血红蛋白(Hb)、转铁蛋白(TRF)、术后吻合口瘘及切口感染发生率、干预前后自我护理能力(ESCA量表)及出院满意度等情况。结果试验组患者PG-SGA评分低于对照组患者(P<0.05),ALb、Hb、TRF水平高于对照组患者(均P<0.05),术后吻合口瘘及切口感染发生率低于对照组(P<0.05),ESCA量表各项评分高于对照组患者(P<0.05),出院满意度高于对照组患者(P<0.05)。结论“互联网+护理”背景下分阶段回授法能有效改善食管癌患者术后营养状况,降低术后吻合口瘘及切口感染率,提高患者自我护理能力,大幅度改善患者满意度。
Objective To investigate the application effect of the phased feedback method under the background of“Internet+Nursing”in the nutritional management of patients undergoing esophageal cancer surgery during the perioperative period.Methods The study selected patients who underwent esophageal cancer surgery in the Department of thoracic surgery of Northen Jiangsu People s Hospital from September 2023 to February 2024.40 patients who underwent surgery from September to November 2023 were set as the control group,and the other 40 patients who underwent surgery from December 2023 to February 2024 were designated as the experimental group.The control group implemented conventional nutritional management measures,while the experimental group implemented the phased feedback method under the background of“Internet+Nursing”based on the conventional nutritional management measures.The patient s subjective comprehensive assessment(PG-SGA),serum albumin(ALb),hemoglobin(Hb),transferrin(TRF),as well as the incidence of anastomotic fistula and wound infection after surgery,self-care ability(ESCA scale),and satisfaction rate of discharge were compared between the two groups.Results The score of PG-SGA in the experimental group was lower,the ALb,Hb,and TRF levels of the experimental group were higher than those of the control group(P<0.05).The incidence of anastomotic fistula and wound infection after surgery in the experimental group was lower than that of the control group(P<0.05);The score of self-care ability and the satisfaction rate with discharge in the experimental group were higher than those of the control group(P<0.05);Conclusions The phased feedback method in the context of“Internet+Nursing”can effectively improve the nutritional status of esophageal cancer patients after surgery,reduce the incidence of anastomotic fistula and wound infection after surgery,enhance patients self-care ability,and significantly improve patient's satisfaction.
