摘要
目的调查女性人类免疫缺陷病毒(human immunodeficiency virus,HIV)感染者生殖道人乳头瘤病毒(human papillomavirus,HPV)感染情况,并分析生殖道HPV感染与阴道微环境的关系。方法选择2022年6月—2023年6月于南京市浦口区妇幼保健计划生育服务中心诊治的女性HIV感染者,检测其生殖道HPV感染情况,评估其阴道微环境状况,采用logistic回归分析女性HIV感染者生殖道HPV感染与阴道微环境状况的关系。结果111例女性HIV感染者中59例生殖道HPV感染,占53.15%。Logistic回归分析结果显示,乳酸杆菌异常(OR=1.662,95%CI:1.154~2.393,P=0.006)、细菌性阴道病阳性(OR=1.826,95%CI:1.191~2.799,P=0.006)、阴道清洁度异常(OR=2.394,95%CI:1.217~4.708,P=0.011)、唾液酸苷酶异常(OR=3.849,95%CI:1.713~8.648,P=0.001)、白细胞酯酶阳性(OR=5.786,95%CI:2.250~14.882,P<0.001)是女性HIV感染者生殖道HPV感染的独立危险因素。结论女性HIV感染者生殖道HPV感染率高,且生殖道HPV感染与阴道微环境密切相关。
Objective To investigate the genital tract human papillomavirus(HPV)infection in female human immunodeficiency virus(HIV)infected patients,and to analyze the relationship between genital tract HPV infection and vaginal microenvironment.Methods From June 2022 to June 2023,female HIV infected patients diagnosed and treated in Nanjing Pukou District Maternal and Child Health and Family Planning Service Center were selected to detect their genital tract HPV coinfection,their vaginal microenvironment were evaluated,and logistic regression was used to analyze the relationship between genital tract HPV infection and vaginal microenvironment in female HIV infected patients.Results Among 111 female HIV infected patients,59(53.15%)of them had genital tract HPV infection.The result of logistic regression analysis showed that abnormal lactobacilli(OR=1.662,95%CI:1.154-2.393,P=0.006),positive bacterial vaginosis(OR=1.826,95%CI:1.191-2.799,P=0.006),abnormal vaginal cleanliness(OR=2.394,95%CI:1.217-4.708,P=0.011),abnormal sialidase(OR=3.849,95%CI:1.713-8.648,P=0.001),and positive leukocyte esterase(OR=5.786,95%CI:2.250-14.882,P<0.001)were independent risk factors for genital tract HPV co-infection in female HIV infected patients.Conclusions The genital tract HPV infection rate is high in female HIV-infected patients,and genital tract HPV co-infection is closely related to vaginal microenvironment.
