摘要
目的比较两种全自动化学发光免疫分析仪在人类免疫缺陷病毒(HIV)抗原抗体检测中的应用价值。方法选取2022年1月至2023年12月在金湖县人民医院体检的18745名健康管理人员作为研究对象,采用罗氏cobas8000和亚辉龙iFlash3000对所采集血液样本进行HIV抗原抗体检测,初筛阳性样本以免疫印迹法确证。结果罗氏cobas8000的所需血清量少于亚辉龙iFlash3000,检验时间短于亚辉龙iFlash3000,差异具有统计学意义(P<0.05)。罗氏cobas8000的HIV抗原抗体阳性检出率为0.41%,高于亚辉龙iFlash3000的0.19%,差异具有统计学意义(P<0.05)。罗氏cobas8000的HIV抗原抗体阳性检测准确度为46.75%,低于亚辉龙iFlash3000的97.40%,差异具有统计学意义(P<0.05)。罗氏cobas8000 HIV抗原抗体检测存在41例假阳性样本,其中97.56%(40/41)分布于临界指数(COI)1.0~10.0,2.44%(1/41)分布于COI>10.0。结论罗氏cobas8000检测HIV抗原抗体的所需血清量少,检验时间短,但假阳性率较高;而亚辉龙iFlash3000的准确度更高,临床可根据具体情况选择检测仪器。
Objective To compare the application value of two automatic chemiluminescence immunoassay analyzers in the detection of human immunodeficiency virus(HIV)antigen and antibody.Methods A total of 18745 health managers who underwent physical examination in Jinhu County People's Hospital from January 2022 to December 2023 were selected as the research objects.Roche cobas8000 and Yahuilong iFlash3000 were used to detect HIV antigen and antibody in the collected blood samples,and the positive screening samples were confirmed by immunoblotting.Results The amount of serum required in the Roche cobas8000 was less than that in the Yahuilong iFlash3000,and the detection time was shorter than that in the Yahuilong iFlash3000,and the differences were statistically significant(P<0.05).The positive detection rate of HIV antigen and antibody in the Roche cobas8000 was 0.41%,which was higher than 0.19%in the Yahuilong iFlash3000,and the difference was statistically significant(P<0.05).The accuracy of HIV antigen and antibody positive detection in the Roche cobas8000 was 46.75%,which was lower than 97.40%in the Yahuilong iFlash3000,and the difference was statistically significant(P<0.05).There were 41 false positive samples in the Roche cobas8000 HIV antigen and antibody detection,of which 97.56%(40/41)were distributed in cut off index(COI)1.0-10.0,and 2.44%(1/41)were distributed in COI>10.0.Conclusion Roche cobas8000 requires less serum to detect HIV antigen and antibody,and the test time is short,but the false positive rate is high.The accuracy of Yahuilong iFlash3000 is higher,and the detection instrument can be selected according to the specific situation.
