心理弹性在妇科恶性肿瘤患者配偶照顾负担和预期性悲伤间的中介效应

Mediating effect of psychological resilience between spousal nursing burden and anticipatory grief in patients with gynaecological malignant tumor

目的探讨心理弹性在妇科恶性肿瘤患者配偶照顾负担和预期性悲伤间的中介效应。方法采用心理弹性量表(CD-RISC)、预期性悲伤量表(AGS)和照顾者负担量表(ZBI)对2022年1—12月江苏省某三级甲等医院肿瘤中心住院的176例妇科恶性肿瘤患者的配偶进行调查,并对调查结果进行统计分析。结果配偶心理弹性得分为62.19±7.11分、预期性悲伤得分为80.55±7.56分、照顾负担得分为46.19±6.58分,均处于中等水平。心理弹性与预期性悲伤、照顾负担均呈负相关(r=-0.778、-0.835,均P<0.01),而预期性悲伤与照顾负担呈正相关(r=0.846,P<0.01)。心理弹性在照顾负担和预期性悲伤间起部分中介效应,中介效应贡献率为23.29%。结论照顾负担和心理弹性均是妇科恶性肿瘤患者配偶预期性悲伤的重要影响因素,心理弹性能够降低照顾负担对预期性悲伤的影响,因此在护理过程中,护理人员应重视妇科恶性肿瘤患者配偶心理弹性的重要性,并采取个性化护理措施来提升妇科恶性肿瘤患者配偶心理弹性水平,降低其照顾负担和预期性悲伤,进而提高患者及其配偶的生活质量以及配偶的照护质量。

Objective To explore the mediating effect of psychological resilience between spousal nursing burden and anticipatory grief in patients with gynaecological malignant tumor.Methods The Connor-Davidson Resilience Scale(CDRISC),the Anticipatory Grief Scale(AGS)and Zarit Caregiver Burden Inventoy(ZBI)scale were used to investigate the spouses of 176 patients with gynaecological malignant tumor admitted to the oncology centre of a Class III Grade A hospital in Jiangsu province from January to December 2022,and the results were statistically analysed.Results The spouses'psychological resilience score is 62.19±7.11 points,anticipatory grief score is 80.55±7.56 points,and caregiving burden score is 46.19±6.58 points were at a moderate level.Psychological elasticity was negatively correlated with anticipatory sadness and caregiving burden(r=-0.778,-0.835,both P<0.01),while anticipatory sadness was positively correlated with caregiving burden(r=0.846,P<0.01).Psychological resilience partially mediated the effect between caregiving burden and anticipatory grief,with a mediating effect contribution of 23.29%.Conclusion Caregiving burden and psychological elasticity are both important influencing factors of anticipatory sadness in spouses of patients with gynaecological malignant tumor,and psychological elasticity could reduce the influence of caregiving burden on anticipatory sadness.The psychological resilience of the spouses of patients with gynaecological malignant tumor should be improved,the burden of care and anticipatory sadness should be reduced,so as to improve the quality of life of the patients and their spouses as well as the quality of spousal care.
