摘要
目的分析尿蛋白、尿微量白蛋白与糖化血红蛋白指标联合检验对糖尿病肾病疾病的诊断价值。方法选择2021年11月—2022年11月乌鲁木齐市中医医院诊治的44例疑似糖尿病肾病患者为研究对象,所有患者均检测尿蛋白、尿微量白蛋白、糖化血红蛋白指标,以临床综合诊断结果为金标准,分析不同检测方法的诊断结果与诊断效能。结果临床综合诊断结果确诊40例阳性,尿蛋白联合糖化血红蛋白确诊35例阳性,尿微量白蛋白联合糖化血红蛋白确诊37例阳性,3种指标联合检测确诊40例阳性。3种指标联合检测的准确度、灵敏度、特异度、阳性预测值和阴性预测值均高于尿蛋白联合糖化血红蛋白、尿微量白蛋白联合糖化血红蛋白检测,差异有统计学意义(P均<0.05)。结论联合检测尿蛋白、尿微量白蛋白与糖化血红蛋白指标可准确反映糖尿病肾病的情况,诊断效能较高,具有辅助临床确诊的价值,有助于发现早期肾病。
Objective To analyze the diagnostic value of combined test of urinary protein,urinary microalbumin and glycated hemoglobin A1c in diabetic nephropathy.Methods From November 2021 to November 2022,44 patients with suspected diabetic nephropathy diagnosed and treated in Urumqi Traditional Chinese Medicine Hospital were selected as the observation objects.All patients were tested for urinary protein,urinary microalbumin and glycated hemoglobin A1c.The clinical comprehensive diagnosis results were used as the gold standard.The diagnostic results and diagnostic efficacy of different examination methods were analyzed.Results Clinical comprehensive diagnosis confirmed 40 positive cases,urinary protein combined with glycated hemoglobin A1c confirmed 35 positive cases,urinary microalbumin combined with glycated hemoglobin A1c confirmed 37 positive cases,and 40 positive cases were confirmed by the combined detection of three indicators.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of the combined detection of the three indicators were higher than those of urinary protein combined with glycated hemoglobin A1c and urinary microalbumin combined with glycated hemoglobin A1c,and the differences were statistically significant(all P<0.05).Conclusion The combined detection of urinary protein,urinary microalbumin and glycated hemoglobin A1c can accurately reflect the situation of diabetic nephropathy,and the diagnostic efficiency is high.It has the value of assisting clinical diagnosis and is helpful to find early nephropathy.
