点阵CO_(2)激光联合曲安奈德外用治疗增生性瘢痕的疗效观察

Efficacy of Fractional CO_(2)Laser Combined with Topical Triamcinolone Acetonide in the Treatment of Hypertrophic Scars

目的:观察点阵CO_(2)激光联合曲安奈德对增生性瘢痕的治疗效果。方法:选取2022年10月-2023年10月笔者医院共收治86例增生性瘢痕患者,随机分为对照组和观察组,各43例。对照组接受点阵CO_(2)激光治疗,观察组在此基础上添加曲安奈德治疗。比较两组在瘢痕恢复情况[瘙痒评分和温哥华瘢痕量表(VSS)评分、视觉模拟评分法(VAS)评分]、生活质量[睡眠质量(PSQI)和皮肤病生活质量指标调查表(DLQI)],以及不良反应和复发率。结果:治疗后,相较于对照组,观察组瘙痒评分、VSS评分和VAS评分均明显较低(P<0.05),PSQI和DLQI评分均明显较低(P<0.05);两组不良反应发生率无明显差异(P>0.05),观察组复发率明显低于对照组(P<0.05)。结论:点阵CO_(2)激光联合曲安奈德治疗增生性瘢痕疗效显著,可有效改善瘢痕皮损情况,提高患者的生活质量,明显降低复发率。

Objective This study aimed to evaluate the therapeutic efficacy of fractional CO_(2)laser combined with triamcinolone acetonide for the treatment of hypertrophic scars.Methods From October 2022 to October 2023,a total of 86 patients withhypertrophic scars were treated at the author’s hospital and were randomly assigned to a control group or an observation group,with 43 patients in each group.The control group received fractional CO_(2)laser treatment,while the observation group received fractional CO_(2)laser treatment combined with triamcinolone acetonide.The two groups were compared in terms of scar recovery[itch score,Vancouver scar scale(VSS)score,visual analog scale(VAS)score],quality of life[Pittsburgh Sleep Quality Index(PSQI)and dermatology life quality index(DLQI)],as well as adverse effects and recurrence rates.Results After treatment,the observation group had significantly lower pruritus scores,VSS scores,and VAS scores compared to the control group(P<0.05).The PSQI and DLQI scores were also significantly lower in the observation group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups,but the recurrence rate in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Fractional CO_(2)laser combined with triamcinolone acetonide demonstrates significant efficacy in the treatment of hypertrophic scars.This combination can effectively enhance scar recovery,improve patients’quality of life,and significantly reduce the recurrence rate.
