摘要
目的分析糖尿病视网膜病变(diabetic retinopathy,DR)严重程度与血清摄食抑制因子(nesfatin-1)和爱帕琳肽(apelin)、血红素氧合酶-1(heme oxygenase-1,HO-1)水平及血生化指标的相关性。方法选择2022年1月—2023年6月福建医科大学附属泉州市第一医院眼科接收的96例2型糖尿病患者,根据DR严重程度分为6期,分别为Ⅰ期26例、Ⅱ期25例、Ⅲ期18例、Ⅳ期14例、Ⅴ期9例、Ⅵ期4例,另以16名健康受检者为对照组,检测血生化指标、nesfatin-1、apelin、HO-1水平,并分析其与DR严重程度的相关性。结果DR患者血生化指标(空腹血糖、餐后2 h血糖、糖化血红蛋白、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、甘油三酯)水平与对照组比较,差异有统计学意义(P均<0.05)。不同DR严重程度患者的空腹血糖、餐后2 h血糖、糖化血红蛋白、低密度脂蛋白胆固醇、甘油三酯、高密度脂蛋白胆固醇水平比较,差异有统计学意义(P均<0.05)。DR患者的nesfatin-1、HO-1水平均低于对照组,apelin水平高于对照组,差异有统计学意义(P均<0.05)。不同DR严重程度患者的nesfatin-1、HO-1、apelin水平比较,差异有统计学意义(P均<0.05)。DR患者nesfatin-1、HO-1、高密度脂蛋白胆固醇与DR分期呈负相关,apelin、空腹血糖、餐后2 h血糖、糖化血红蛋白、低密度脂蛋白胆固醇、甘油三酯与DR分期呈正相关。结论nesfatin-1、apelin、HO-1水平、血糖血脂各项血生化指标均与DR严重程度存在相关性。
Objective To analyze the correlation between the severity of diabetic retinopathy(DR)and the levels of serum nesfatin-1,apelin,heme oxygenase-1(HO-1),blood biochemical indexes.Methods A total of 96 patients with type 2 diabetes mellitus who were admitted to the Department of Ophthalmology of Quanzhou First Hospital Affiliated to Fujian Medical University from January 2022 to June 2023 were selected.According to the severity of DR,they were divided into 6 stages,26 cases in stageⅠ,25 cases in stageⅡ,18 cases in stageⅢ,14 cases in stageⅣ,9 cases in stageⅤ,and 4 cases in stageⅥ.In addition,16 healthy subjects were used as the control group.The levels of blood biochemical indexes,nesfatin-1,apelin and HO-1 were detected,and the correlation with the severity of DR was analyzed.Results The levels of blood biochemical indexes(fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,high density lipoprotein cholesterol,low density lipoprotein cholesterol,triglyceride)in DR patients and the control group were compared,and the differences were statistically significant(all P<0.05).There were statistically significant differences in the levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,low density lipoprotein cholesterol,triglyceride and high density lipoprotein cholesterol in patients with different DR severity(all P<0.05).The levels of nesfatin-1 and HO-1 in DR patients were lower than those in the control group,and apelin was higher than that in the control group,and the differences were statistically significant(all P<0.05).There were statistically significant differences in the levels of nesfatin-1,HO-1 and apelin in patients with different DR severity(all P<0.05).In DR patients,nesfatin-1,HO-1 and high-density lipoprotein cholesterol were negatively correlated with the stage of DR,while apelin,fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,low density lipoprotein cholesterol and triglyceride were positively correlated with the stage of DR.Conclusion The levels of nesfatin-1,apelin,HO-1,blood glucose and blood lipid indexes were correlated with the severity of DR.
