ICU糖尿病酮症酸中毒患者治疗中胰岛素不同给药方式的应用效果

摘要 目的探究重症监护室(intensive care unit,ICU)糖尿病酮症酸中毒患者应用胰岛素不同给药方式的作用效果。方法回顾性选取2023年4月—2024年5月上海市第六人民医院福建医院收治的88例ICU糖尿病酮症酸中毒患者的临床资料,根据不同治疗方式分为对照组(n=44,多次静脉滴注小剂量胰岛素)、观察组(n=44,微量输液泵持续泵入胰岛素)。比较两组临床疗效、糖代谢指标、不良反应发生率。结果观察组治疗总有效率高于对照组,治疗15 d后的餐后2 h血糖、空腹血糖、糖化血红蛋白水平以及不良反应发生率均低于对照组,差异有统计学意义(P均<0.05)。结论ICU糖尿病酮症酸中毒注射胰岛素时选择微量输液泵持续泵入方式,可提高治疗有效率,增强控糖效果,降低不良反应的发生。 Objective To explore the effect of different administration methods of insulin in patients with diabetic ketoacidosis in intensive care unit(ICU).Methods The clinical data of 88 patients with diabetic ketoacidosis in ICU admitted to Shanghai Sixth People's Hospital Fujian Hospital from April 2023 to May 2024 were retrospectively selected.According to different treatment methods,they were divided into control group(n=44,multiple intravenous infusion of low-dose insulin)and observation group(n=44,continuous infusion of insulin by micro-infusion pump).The clinical efficacy,glucose metabolism indexes and incidence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the levles of 2-hour postprandial blood glucose,fasting blood glucose,glycated hemoglobin A1c after 15 days of treatment and incidence of adverse reactions were lower than those in the control group,the differences were statistically significant(all P<0.05).Conclusion When insulin is injected into diabetic ketoacidosis in ICU,continuous pumping of micro-infusion pump can improve the effective rate of treatment,enhance the effect of glucose control and reduce the occurrence of adverse reactions.
