血清Eotaxin、PAF预测急性大出血输血相关性急性肺损伤患者预后价值研究

摘要 目的分析血清嗜酸性粒细胞趋化因子(serum eosinophil chemokine,eotaxin)、血小板活化因子(platelet activating factor,PAF)预测急性大出血输血相关性急性肺损伤患者的预后价值。方法回顾性选取2022年1月—2024年1月济宁市兖州区人民医院收治的205例急性大出血输血相关性急性肺损伤患者的病历资料。根据随访预后的情况,分为预后不良组(n=19)、预后良好组(n=186)。采用单因素、多因素Logistic回归模型分析血清Eotaxin、PAF对急性大出血输血相关性急性肺损伤患者预后的影响。绘制ROC曲线并计算AUC,探讨血清Eotaxin、PAF对预测急性大出血输血相关性急性肺损伤患者预后价值。结果经多因素Logistic回归结果显示,血清Eotaxin、PAF是急性大出血输血相关性急性肺损伤患者预后的影响因素。绘制ROC曲线发现,血清Eotaxin、PAF预测急性大出血输血相关性急性肺损伤患者的AUC分别为0.770、0.827。结论血清Eotaxin、PAF可作为急性大出血输血相关性急性肺损伤患者预后的潜在生物标志物,具有一定的预测价值。 Objective To analyze the prognostic value of serum eosinophil chemokine(eotaxin)and platelet activating factor(PAF)in patients with acute massive hemorrhage and transfusion related acute lung injury.Methods The medical records of 205 patients with acute massive hemorrhage and transfusion-related acute lung injury admitted to Yanzhou District People′s Hospital of Jining City from January 2022 to January 2024 were retrospectively selected.According to the prognosis of follow-up,they were divided into poor prognosis group(n=19)and good prognosis group(n=186).Univariate and multivariate Logistic regression models were used to analyze the effects of serum Eotaxin and PAF on the prognosis of patients with acute massive hemorrhage transfusion-related acute lung injury.The ROC curve was drawn and AUC was calculated to explore the prognostic value of serum Eotaxin and PAF in patients with acute massive hemorrhage and blood transfusion-related acute lung injury.Results Multivariate Logistic regression results showed that serum Eotaxin and PAF were the influencing factors for the prognosis of patients with acute massive hemorrhage and transfusion-related acute lung injury.ROC curve showed that the AUC of serum Eotaxin and PAF in predicting the prognosis of patients with acute massive hemorrhage and transfusion-related acute lung injury were 0.770 and 0.827,respectively.Conclusion Serum Eotaxin and PAF may be used as potential biomarkers for the prognosis of patients with acute massive hemorrhage and transfusion-related acute lung injury,and have certain predictive value.
