摘要
目的 探究血清β2-微球蛋白(β2-microglobulin, β2-MG)、乳酸脱氢酶(lactate dehydrogenase, LDH)、C反应蛋白(C-reactive protein, CRP)、铁蛋白(serum ferritin, SF)、白蛋白(albumin, Alb)检测对恶性血液病的诊断价值。方法 非随机选取2020年1月—2024年3月扬州大学附属靖江人民医院收治的70例血液病患者作为研究对象,按照疾病类型不同分为恶性组(40例)和良性组(30例),同时选取20名健康体检者作对照组,3组患者均予以血清学检测。比较恶性组、良性组和对照组的血清β2-MG、LDH、CRP、SF和Alb水平,并使用ROC曲线分析血清β2-MG、LDH、CRP、SF、Alb单一指标和联合指标检测恶性血液病的诊断效能。结果 恶性组血清β2-MG、LDH、CRP、SF水平均高于良性组,差异有统计学意义(P均<0.05)。良性组血清LDH、CRP、SF、Alb水平均高于对照组,差异有统计学意义(P均<0.05)。恶性组患者血清β2-MG、LDH、CRP、SF、Alb水平均高于对照组,差异有统计学意义(P均<0.05)。血清β2-MG、LDH、CRP、SF、Alb联合检测的AUC值为0.932,其诊断较能高于单一指标的检测。结论 血清β2-MG、LDH、CRP、SF、Alb联合检测在血液病良、恶性诊断中具有较高借鉴价值,可作为恶性疾病诊断的重要参考数据。
Objective To explore the diagnostic value of serumβ2-microglobulin(β2-MG),lactate dehydrogenase(LDH),C-reactive protein(CRP),serum ferritin(SF)and albumin(Alb)in hematological malignancies.Methods From January 2020 to March 2024,seventy patients with hematological diseases admitted to Jingjiang People′s Hospital Affiliated to Yangzhou University were non-randomly selected as the research objects.According to the different types of diseases,they were divided into malignant group(forty cases)and benign group(thirty cases).At the same time,twenty healthy subjects were selected as the control group.All three groups of patients were given serological tests.The levels of serumβ2-MG,LDH,CRP,SF and Alb in the malignant group,the benign group and the control group were compared,and the ROC curve method was used to analyze the diagnostic efficacy of serumβ2-MG,LDH,CRP,SF,Alb single index and combined index in the detection of malignant hematological diseases.Results The levels of serumβ2-MG,LDH,CRP and SF in the malignant group were higher than those in the benign group,and the differences were statistically significant(all P<0.05).The levels of serum LDH,CRP,SF and Alb in the benign group were higher than those in the control group, and the differences were statistically significant (all P<0.05). The levels of serum β2-MG, LDH, CRP, SF and Alb in the malignant group were higher than those in the control group, and the differences were statistically significant (all P<0.05). The AUC value of combined detection of serum β2-MG, LDH, CRP, SF and Alb was 0.932, which was higher than that of single index. Conclusion The combined detection of serum β2-MG, LDH, CRP, SF and Alb has high reference value in the diagnosis of benign and malignant hematological diseases, and can be used as an important reference data for the diagnosis of malignant diseases.
