基于Ai-PCA系统的全程镇痛管理对老年髋部骨折患者术后疼痛情况的影响研究

摘要 目的:探讨基于智能化精准自控镇痛(Ai-PCA)系统的全程镇痛管理对老年髋部骨折患者术后疼痛情况的影响。方法:选取2023年3月—2024年1月宁夏医科大学总医院收治的80例老年髋部骨折患者为研究对象,按随机数字表法将其分为对照组和研究组,各40例。对照组术后实施常规疼痛管理,研究组术后实施基于Ai-PCA系统的全程镇痛管理。比较两组的镇痛效果、术后镇痛情况、不良反应发生情况、慢性疼痛发生情况及髋关节功能恢复情况。结果:术后12、24、48 h,研究组的疼痛评分均低于对照组,差异均有统计学意义(P<0.05);术后48 h,研究组舒芬太尼使用剂量及电子镇痛泵总按压次数少于对照组,研究组电子镇痛泵有效按压次数多于对照组,差异均有统计学意义(P<0.05);术后住院期间,研究组不良反应发生率为5.00%,低于对照组的25.00%,差异有统计学意义(P<0.05);术后3个月,研究组的慢性疼痛发生率为2.50%,低于对照组的20.00%,差异有统计学意义(P<0.05);术后3个月,研究组Harris髋关节功能评分量表各方面评分均高于对照组,差异有统计学意义(P<0.05)。结论:基于Ai-PCA系统的全程镇痛管理可提升老年髋部骨折患者的术后镇痛效果,减少镇痛药物使用剂量,降低不良反应发生率、慢性疼痛发生率,有效促进患者髋关节功能康复。 Objective:To explore the effect of whole course analgesia management based on Artificial Intelligence-Patient Controlled Analgesia(Ai-PCA)system on postoperative pain in elderly patients with hip fracture.Methods:A total of 80 elderly patients with hip fracture admitted to the General Hospital of Ningxia Medical University from March 2023 to January 2024 were selected as the study objects.They were divided into a control group and a study group by random number table method,with 40 cases in each group.The control group received routine pain management after surgery,and the study group received whole course analgesia management based on Ai-PCA system after surgery.The analgesic effect,postoperative analgesia,occurrence of adverse reactions,occurrence of chronic pain and recovery of hip function were compared between the two groups.Results:At the 12,24 and 48 h after surgery,the pain scores of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).Forty-eight hours after surgery,the dosage of Sufentanil and the total number of electronic analgesic pump compressions in the study group were less than those in the control group,the effective times of electronic analgesic pump compressions in the stioly group were more than those in the control group,with statistical significance(P<0.05).During postoperative hospitalization,the incidence of adverse reactions in the study group was 5.00%,which was lower than 25.00%in the control group,with statistical significance(P<0.05).Three months after surgery,the incidence of chronic pain in the study group was 2.50%,which was lower than 20.00%in the control group,the difference was statistically significant(P<0.05).At 3 months after surgery,the scores of all aspects in Harris Hip Function Scale of the study group were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion:The whole process analgesic management based on Ai-PCA system can improve the postoperative analgesic effect in elderly patients with hip fracture,reduce the dosage of analgesic drugs,reduce the incidence of adverse reactions and chronic pain,effectively promote the functional rehabilitation of patients with hip fracture.
