李延教授治疗痞满临床用药规律研究

Study on Clinical Medication Pattern of Professor Li Yan's in Treatment of Fullness Disease

目的:通过对《李延临床医案选》中类属痞满疾病的辨证及用药特点进行分析,深入挖掘李延教授治疗痞满的辨证用药特点,学习其诊疗经验,以指导中医临床。方法:收集整理《李延临床医案选》中用于治疗痞满疾病的方剂,通过中医传承辅助平台软件(V 2.5)进行频数分析,确定高频药物,并对其药性、药味、归经及常用配伍进行归纳总结。结果:共纳入中药555味,治疗痞满使用频次最多的单味中药是茯苓,常用中药以清热解毒、润肠通便、燥湿化痰为主,配伍以行气活血、利水渗湿调和诸药;药性以温、平、微寒为主;药味以甘、苦、辛为主;归经主要归脾、胃、肝经等。结论:李延教授治疗痞满时,以补气健脾、燥湿化痰、清热解毒,兼行气活血、调和阴阳为基本法则,味甘、苦、辛,性温、平、微寒,归脾、肺、肝经之药可作为首选。李延教授临证经验可为临床治疗消化系统相关疾病提供参考。

Objective:To explore Professor Li Yan's dialectical and drug use characteristics in the treatment of Puppimanian diseases,learn his experience in diagnosis and treatment,and guide the clinical treatment of puppimanian diseases by analyzing the dialectical and drug use characteristics in Li Yan's Clinical Medical Records.Methods:The prescriptions used in the treatment of ppizman disease in Li Yan's Clinical Medical Records were collected and sorted out.The frequency analysis was carried out through the traditional Chinese medicine inheritance auxiliary platform software(V2.5)to determine the high-frequency drugs,and their drug properties,drug taste,classics and common compatibility were analyzed.Results:555 traditional Chinese medicines were included.Poria was the most frequently used single traditional Chinese medicine in the treatment of fullness disease.The common traditional Chinese medicines were mainly clearing heat and detoxifying,moistening bowel and removing phlegm,and drying dampness and eliminating phlegm.The drug properties were mainly warm,flat and slightly cold.The taste of medicine is mainly sweet,bitter and xin.The main channels are spleen,stomach and liver.Conclusion:Professor Li Yan used invigorating qi and invigorating spleen,drying dampness and resolving phlegm,clearing heat and detoxifying,activating qi and activating blood,and harmonizing Yin and Yang as the basic rules,sweet,bitter,xin,warm,flat and slightly cold,and the medicine for regulating spleen,lung and liver meridian can be used as the first choice,providing reference for the clinical treatment of digestive system related diseases.
