维持性血液透析患者生活质量现状及其影响因素

Quality of life and its influencing factors in maintenance hemodialysis patients

目的:了解沈阳市3家医院维持性血液透析(MHD)患者的生活质量(QOL),分析QOL的影响因素,为改善MHD患者的QOL提供参考。方法:选择2021年3月至10月3家医院收治的MHD患者343例为研究对象,收集其一般资料、实验室指标、血液透析指标等,并采用KDQOL-SFTM1.3量表(SF-36评分、KDTA评分)对患者QOL进行评分。采用多元线性回归分析影响MHD患者QOL的因素。结果:MHD患者SF-36得分(51.22±19.50)分、KDTA得分(60.43±14.50)分。多元线性回归分析结果显示,年龄、婚姻状况、家庭人均月收入、血管通路、单核细胞计数、血磷、血清白蛋白是SF-36评分的独立影响因素(P<0.05);年龄、文化程度、家庭人均月收入、血清肌酐是KDTA评分的独立影响因素(P<0.05)。结论:需重视对影响因素的综合干预,以提高MHD患者生活质量。

Objective:To investigate the quality of life(QOL)of patients undergoing maintenance hemodialysis(MHD)in blood purification centers of three hospitals in Shenyang,and analyze its influencing factors,so as to provide reference for improving QOL of MHD patients.Methods:A total of 343 patients undergoing MHD from Mar to Oct,2021 were included,and their general data,laboratory indicators,hemodialysis indicators,etc.were collected.QOL of patients was evaluated by KDQOL-SFTM1.3 scale(SF-36 score and KDTA score).Multivariable linear regression analysis was used to analyze the factors affecting their QOL.Results:The scores of SF-36 and KDTA in MHD patients were 51.22±19.50 and 60.43±14.50,respectively.Age,marital status,family per capita monthly income,vascular access,monocyte count,blood phosphorus and serum albumin were independent influencing factors of SF-36 score(P<0.05).And age,education level,family per capita monthly income and serum creatinine were independent influencing factors of KDTA score(P<0.05).Conclusion:Comprehensive intervention of influencing factors should be paid more attention to improve the QOL of MHD patients.
