数字化智慧医疗模式提高维持性血透患者血红蛋白达标率的研究

Application Research of Quality Control Circle in Improving the Hemoglobin Compliance Rate of Maintenance Hemodialysis Patients in the Digitalized Smart Healthcare Framework

摘要 目的:本研究旨在探讨品管圈在数字化智慧医疗模式中提高维持性血透患者血红蛋白达标率的效果。方法:选取苏州大学附属第一医院血透中心2022年10月8日至10月30日期间维持性血液透析患者140例,采用品管圈十步手法进行管理,于2023年2-3月再次查检血透患者血红蛋白,并进行统计分析。结果:现况调查中,41.4%的患者血红蛋白达标,不达标的主要原因包括未及时检查和治疗不及时,进一步通过真因验证,发现信息交互不通畅、缺少居家自我管理监测方案、缺少用药与治疗管理流程、患者贫血预警不及时为血红蛋白不达标的真因,经过品管圈管理后,血红蛋白达标率提高至69.29%,与改善前相比差异具有统计学意义(χ^(2)=21.98,P<0.001)。结论:品管圈在数字化智慧医疗模式中能有效提高维持性血液透析患者的血红蛋白达标率,对优化治疗和提高患者生活质量具有重要意义,值得在其他医疗领域进一步推广和应用。 Objective:This study aimed to assess the efficacy of quality control circle(QCC)in enhancing the hemoglobin standardization rate among maintenance hemodialysis patients within a digitalized smart healthcare framework.Methods:A cohort of 140 maintenance hemodialysis patients from the Hemodialysis Center of the First Affiliated Hospital of Soochow University was selected between October 8 and October 30,2022.Utilizing the ten-step method,QCC were employed for management.Subsequently,hemoglobin levels were reassessed for these patients in February-March 2023,followed by rigorous statistical analysis.Results:Initial assessment revealed that only 41.4%of patients achieved hemoglobin standardization.Common reasons for non-standardization included delayed examinations and inadequate treatment.Root cause analysis identified factors such as ineffective information exchange,absence of home self-management monitoring schemes,deficient medication and treatment management protocols,and untimely anemia alerts as primary contributors to non-standard hemoglobin levels.Following QCC implementation,the hemoglobin standardization rate significantly increased to 69.29%,demonstrating a statistically significant difference compared to the pre-intervention period(χ^(2)=21.98,P<0.001).Conclusion:The findings underscore the effectiveness of QCC within a digitalized smart healthcare model in improving hemoglobin standardization rates among maintenance hemodialysis patients.These results hold considerable implications for treatment optimization and enhancement quality of life,suggesting the worth of further promotion and application in other medical fields.
