磁共振弥散加权成像动态对比增强及脂肪定量技术在强直性脊柱炎骶髂关节病变活动性评价中的价值

The value of diffusion weighted imaging,dynamic contrast enhancement and fat quantitative technique in evaluating the activity of sacroiliac joint lesions in ankylosing spondylitis

目的探讨磁共振弥散加权成像(DWI)、动态对比增强(DCE)及脂肪定量在强直性脊柱炎(AS)骶髂关节病变活动性评价中的价值。方法选取AS骶髂关节病变患者62例,全部患者均接受DWI、DCE及脂肪定量技术检查,比较活动期与稳定期患者的各项结果,同时分析Bath强直性脊柱炎疾病活动性指数(BASDAI)、C反应蛋白(CRP)、红细胞沉降率(ESR)与各影像参数的相关性。结果活动期患者BASDAI、ESR、CRP、表观弥散系数(ADC)、细胞外间隙容积与血管内容积比、对比度传递系数、细胞外间隙容积值更高,脂肪分数值更低(P<0.05);BASDAI、ESR、CRP与ADC、细胞外间隙容积与血管内容积比、对比度传递系数、细胞外间隙容积、脂肪分数值存在相关性(P<0.05)。结论DWI、DCE及脂肪定量技术可有效评价AS骶髂关节病变活动性。

Objective To investigate the value of diffusion weighted imaging(DWI),dynamic contrast enhancement(DCE)and fat quantification techniques in evaluating the activity of sacroiliac joint lesions in ankylosing spondylitis(AS).Methods A total of 62 patients with AS sacroiliac joint lesions were selected,all of which were examined by DWI,DCE-MRI and fat quantitative techniques.The results of active and stable patients were compared,and the correlation of Bath ankylosing spondylitis disease activity index(BASDAI),C-reactive protein,erythrocyte sedimentation rate and each image parameter were analyzed.Results The disease activity index,erythrocyte sedimentation rate,C-reactive protein,apparent diffusion coefficient,ratio of extracellular space volume to vascular content volume,contrast transfer coefficient,extracellular space volume value of Bath ankylosing spondylitis in active patients were higher,and fat fraction value was lower(P<0.05).BASDAI index,erythrocyte sedimentation rate,C-reactive protein and apparent diffusion coefficient,extracellular space volume to vascular content ratio,contrast transfer coefficient,extracellular space volume,fat fraction value were correlated(P<0.05).Conclusion DWI,DCE-MRI and fat quantification techniques can effectively evaluate the activity of sacroiliac joint lesion in AS.
