基于257例偏头痛病人头痛日记数据探讨头痛VAS评分的影响因素

Influencing Factors of Headache VAS Score Based on Headache Diary Data of 257 Migraine Patients

目的:基于257例偏头痛病人1446例次头痛日记数据,探究影响头痛视觉模拟评分法(VAS)评分的因素,以期为偏头痛临床防治提供指导。方法:选取“脑安滴丸治疗偏头痛适应症状群”研究中完整记录头痛日记的病人257例,共记录头痛日记1446例次,分析头痛发作当日的天气、心情、部位、性质、伴随症状、诱因对VAS评分的影响。结果:头痛为“胀痛”“昏沉感头痛”“跳痛”“刺痛”较“隐痛”和“其他类型”头痛的VAS评分更高,差异均有统计学意义(P<0.05);有伴随症状较无伴随症状的VAS评分更高,差异有统计学意义(P<0.05);诱因为工作压力大、生活劳累、生气或着急较无诱因的VAS评分更高,差异有统计学意义(P<0.05);头痛当天“心情不好”较“心情好”和“心情一般”的VAS评分高,差异有统计学意义(P<0.05);不同天气状态及不同头痛部位的VAS评分比较差异无统计学意义(P>0.05)。结论:头痛日记是用于评估偏头痛发作及治疗的有效工具,头痛性质、伴随症状、发作诱因及情绪都是影响头痛VAS评分的重要因素。

Objective:Based on the data of 1446 headache diaries of 257 migraine patients,the factors affecting headache Visual Analogue Scale(VAS)were explored.Methods:A total of 1446 headache diaries were recorded in 257 patients in the study of"Naoan Dropping Pills for Treating Migraine Adaptation Symptom Group".The effects of weather,mood,location,nature,accompanying symptoms,and inducement on VAS score on the day of headache attack were analyzed.Results:The VAS scores of"swelling","faintness","throbbing",or"tingling"were higher than those of"dull pain"and"other headache types",and the differences were statistically significant(P<0.05).The VAS scores of patients with concomitant symptoms were higher than those without symptom,and the difference was statistically significant(P<0.05).The VAS scores of high work pressure,tired life,and feeling angry or anxious were higher than those without incentive,and the difference was statistically significant(P<0.05).The VAS scores of"bad mood"were higher than those with"good mood"and"average mood"on the day of headache,and the difference were statistically significant(P<0.05).There were no significant difference in VAS scores of different weather conditions and headache sites(P>0.05).Conclusion:Headache diary is an effective tool to evaluate the attack and treatment of migraine.The nature of headache,accompanying symptoms,attack triggers,and mood are important factors affecting the score of headache VAS.
