临床医学硕士专业学位研究生心理现状及影响因素分析

Psychological Status of Postgraduates of Professional Master's Degree in Clinical Medicine and Its Influencing Factors

目的了解临床医学硕士专业学位研究生(临床专硕)“四证合一”培养模式下的心理现状,分析影响因素,以提升临床专硕心理健康水平。方法采用方便取样方法,于2024年2月选取四川省4所医学院校1087名临床专硕为研究对象,采用一般资料调查表、一般心理健康问卷(GHQ-12)、压力知觉量表(PSS-4)和抑郁筛查量表(PHQ-2)进行调查,分析临床专硕心理状况及其影响因素,并提出促进临床专硕心理健康的对策。结果93.84%(1020/1087)的临床专硕GHQ-12总分>27分,86.66%(942/1087)的临床专硕PSS-4总分>10分,表明临床专硕普遍承受较高压力,且年级越高压力水平越高,其压力主要来源于学业、前途及就业、临床医疗环境和经济等多个方面;临床专硕抑郁症状普遍较轻,但11.50%(125/1087)的学生抑郁症状较严重(PHQ-2总分>3分),且定向与非定向临床专硕间抑郁症状比较,差异有统计学意义(P<0.05)。结论临床专硕面临严峻的心理健康挑战,高年级学生普遍存在高压力水平;定向临床专硕心理健康问题更突出,需加强关注与干预。

Objective To investigate the psychological status of postgraduates of professional master's degree in clinical medicine under the"four certificates in one"training mode,and analyze the influencing factors,so as to improve the psychological health.Methods A total of 1087 postgraduates of professional master's degree in clinical medicine from four medical colleges and universities in Sichuan Province were selected as the study subjects by convenience sampling method.The General Health Questionnaire(GHQ-12),Perceived Stress Scale(PSS-4),and 2-item version of the Patient Health Questionnaire depression module(PHQ-2)were administered to conduct the survey.The influencing factors of the psychological status were statistically analyzed,and the countermeasures to promote the psychological status were put forward.Results The survey results showed that 93.84%(1020/1087)of the postgraduates scored more than 27 points in GHQ-12,and 86.66%(942/1087)scored more than 10 points in PSS-4,indicating that most of them were facing excessive stress.The stress mainly came from academic pressure,career expectations,medical environment and economic pressure.The higher the grade,the greater the stress.Although the overall depressive symptoms were mild,11.50%(125/1087)of the students showed higher frequency and severity of depressive symptoms(total score of PHQ-2>3).There were significant differences in depressive symptoms between students from a national compulsory service programme(CSP)and non-CSP students(P<0.05).Conclusion Most postgraduates of professional master's degree in clinical medicine are under great pressure and face severe mental health challenges,especially the senior ones and the CSP ones,which require more attention and timely intervention.
