解郁调神针法联合顺气化痰方对脑卒中后抑郁伴失眠的影响

Jieyu Tiaoshen Acupuncture Combined with Shunqi Huatan Prescription in the Treatment of Post-stroke Depression with Insomnia

目的观察解郁调神针法联合顺气化痰方治疗脑卒中后抑郁伴失眠的疗效。方法采用随机数字表法将2022年1月—2023年6月淄博市淄川区医院收治的66例脑卒中后抑郁伴失眠患者分为对照组和观察组,各33例。对照组口服米氮平片,观察组给予解郁调神针法联合顺气化痰方。对比2组疗效。结果观察组总有效率明显高于对照组(P<0.05)。治疗后,观察组PHQ-9评分、PSQI评分、潜伏期、血清中IL-1β水平低于对照组(P<0.05),CD-RISC评分、波幅、5-HT、NPY水平高于对照组(P<0.05);2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论解郁调神针法联合顺气化痰方利于改善脑卒中后抑郁伴失眠患者的抑郁状态、睡眠质量、心理弹性、神经功能,调控神经递质、降低炎性因子表达,值得推广。

Objective To observe the curative effect of Jieyu Tiaoshen acupuncture combined with Shunqi Huatan prescription in the treatment of post-stroke depression with insomnia.Methods A total of 66 patients with post-stroke depression and insomnia admitted to Zichuan District Hospital from January 2022 to June 2023 were divided into control group and observation group,with 33 cases in each group.The control group was treated with mirtazapine tablet,and the observation group was treated with Jieyu Tiaoshen acupuncture combined with Shunqi Huatan prescription.The efficacy of the two groups was compared.Results The effective rate of the observation group was significantly higher than that of the control group(P<0.05),the PHQ-9 score,PSQI score,latency and serum IL-1βlevel after treatment were lower than those of the control group(P<0.05),and the CD-RISC score,amplitude,5-HT and NPY levels were higher than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion Jieyu Tiaoshen acupuncture combined with Shunqi Huatan prescription can improve the depression,sleep quality,mental resilience,nervous function,regulate neurotransmitters and reduce the expression of inflammatory factors in patients with post-stroke depression accompanied by insomnia,which is worthy of promotion.
