摘要
目的探讨特发性正常压力脑积水(idiopathic normal pressure hydrocephalus,iNPH)与常见血管性危险因素(Vascular risk factors,VRF)的关系,并研究病程及严重程度与iNPH的相关性。方法收集我院2019年1月~2022年10月i NPH住院患者104例(i NPH组),以及与iNPH患者年龄、性别和文化程度匹配的认知功能正常的70例体检者(对照组)。收集两组年龄、性别、高血压病、糖尿病、高胆固醇血症、高同型半胱氨酸血症、吸烟、饮酒史等资料。多因素非条件Logistic回归研究VRF与iNPH的关联性,Spearman相关分析研究危险因素、严重程度与iNPH相关性。结果两组年龄、性别(男)比较,差异均无统计学意义(P>0.05),糖尿病、高血压、高胆固醇血症发生率比较,差异有统计学意义(P<0.05)。Logistic回归分析显示,高血压和糖尿病是iNPH发生的危险因素。iNPH与高血压病程、高血压分级、糖尿病心血管危险分层呈正相关。结论iNPH常见的VRF包括高血压、糖尿病,iNPH与高血压病病程、高血压分级、糖尿病心血管危险分层呈正相关。
Objective To explore the correlation between idiopathic normal pressure hydrocephalus(iNPH)and common vascular risk factors(VRF),and investigate the correlation between the duration,severity of related risk factors and iNPH.Methods 104 inpatients with iNPH from Jan 2019 to Oct 2022 in our hospital(iNPH group),and 70 subjects with normal cognitive function matched with the age,gender and education level of iNPH patients were collected(control group).Information on age,gender,hypertension,diabetes,hypercholesterolemia,hyperhomocysteinemia,smoking,drinking history and so on were collected.Multivariate unconditional Logistic regression analysis was used to study the correlation between VRF and iNPH.Spearman correlation analysis were used to study the correlation between the severity of risk factors and iNPH.Results There were no significant differences in age and gender(male)between the iNPH group and the control group(P>0.05).There were significant differences in diabetes,hypertension,and hypercholesterolemia(P<0.05).Multivariate Logistic regression analysis showed that hypertension and diabetes were the risk factors for iNPH.iNPH was positively correlated with the duration and grade of hypertension,and cardiovascular risk stratification of diabetes.Conclusion Hypertension and diabetes are common vascular risk factors for iNPH.iNPH is positively correlated with the duration of hypertension,hypertension classification and cariovascular risk stratification of diabetes.
