摘要
目的探讨甲状腺功能正常的2型糖尿病(T2DM)患者甲状腺激素与血尿酸(SUA)、同型半胱氨酸(Hcy)水平及颈动脉内中膜增厚的关系。方法选取2023年05月至2024年04月我院收治的甲状腺功能正常的T2DM患者130例作为观察组,同时选取甲状腺功能正常的健康体检者130例作为对照组,比较两组游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素(TSH)、SUA、Hcy和颈动脉内中膜厚度(IMT)的差异,分析观察组不同临床特征患者间上述指标差异,以及IMT≥1.0 mm和<1.0 mm患者间FT3、FT4、TSH、SUA、Hcy的差异,同时分析TSH、SUA、Hcy和IMT的相关性。结果观察组和对照组FT3、FT4和SUA比较差异无统计学意义(P>0.05)。观察组TSH、Hcy和IMT分别为(1.72±0.39)mU/L、(11.95±1.69)mmol/L和(0.88±0.12)mm,均高于对照组(P<0.05)。观察组年龄≥50岁患者IMT为(0.93±0.09)mm,高于年龄<50岁患者(P<0.05)。观察组IMT≥1.0 mm患者年龄、TSH、SUA和Hcy分别为(54.35±9.15)岁、(1.97±0.44)mU/L、(354.00±46.62)mmol/L和(14.16±1.81)mmol/L,高于IMT<1.0 mm患者(P<0.05)。TSH与SUA、Hcy、IMT呈正相关(P<0.05);同时IMT与SUA、Hcy呈正相关(P<0.05)。结论甲状腺功能正常的T2DM患者TSH、Hcy和IMT较正常人群升高,其血清TSH水平与SUA、Hcy及IMT呈正相关,同时,甲状腺功能正常的2型糖尿病患者颈动脉内中膜增厚与年龄增长、SUA、Hcy升高存在相关性。
Objective To investigate the relationship between thyroid hormones and blood uric acid(SUA),homocysteine(Hcy)levels and intima-media thickening of carotid arteries in patients with type 2 diabetes mellitus(T2DM)who have normal thyroid function.Methods 130 T2DM patients with normal thyroid function admitted to our hospital from May 2023 to April 2024 were selected as the observation group,and 130 healthy medical check-ups with normal thyroid function were selected as the control group,and the differences in free triiodothyronine(FT3),free thyroxine(FT4),thyrotropin(TSH),SUA,Hcy and carotid artery Intima-media thickness(IMT),and analyse the differences in the above indexes between patients with different clinical characteristics in the observation group,as well as the differences in FT3,FT4,TSH,SUA,Hcy between patients with IMT≥1.0 mm and<1.0 mm,and also analyse the correlation between TSH,SUA,Hcy and IMT.Results There was no statistically significant difference in the comparison of FT3,FT4 and SUA between the observation and control groups(P>0.05).TSH,Hcy and IMT in the observation group were(1.72±0.39)mU/L,(11.95±1.69)mmol/L and(0.88±0.12)mm,respectively,which were higher than those in the control group(P<0.05).The IMT of patients aged≥50 years in the observation group was(0.93±0.09)mm,which was higher than that of patients aged<50 years(P<0.05).The age,TSH,SUA and Hcy of patients with IMT≥1.0 mm in the observation group were(54.35±9.15)years,(1.97±0.44)mU/L,(354.00±46.62)mmol/L and(14.16±1.81)mmol/L,respectively,which were higher than those of patients with IMT<1.0 mm(P<0.05).The TSH was positively correlated with SUA,Hcy,and IMT were positively correlated(P<0.05);meanwhile,IMT was positively correlated with SUA and Hcy(P<0.05).Conclusions TSH,Hcy and IMT were elevated in T2DM patients with normal thyroid function compared with the normal population,and their serum TSH levels were positively correlated with SUA,Hcy and IMT,as well as a correlation between carotid intima-media thickening in type 2 diabetic patients with normal thyroid function and increasing age,and elevated SUA and Hcy.
