摘要
目的了解基于野战医疗所全流程护理岗位军队护士的卫勤保障能力培训现状与需求,为制定科学合理的培训模式及培训考核机制提供依据。方法2022-08~09月,采用便利抽样法在五大战区中每个战区分别抽取2~3所军队医院护士为研究对象,通过问卷星平台自设问卷进行调查。结果共548名军队护士参与调查,57.85%护士有野战医疗所工作经历,55.66%护士参加过轮科培训,26.28%护士有野战医疗所全流程展开7个模块35项卫勤保障能力内容的培训经历;培训途径主要为医院公共培训(59.85%)、专项任务培训(41.97%);培训形式主要为理论授课(91.79%)、操作练习(88.50%)、情景模拟训练(55.66%);培训时间以业余学习(41.24%)为主;考核形式主要为理论考核(76.28%)和操作考核(84.49%);82.66%护士认为现有的护理卫勤培训存在问题,主要为工作与训练时间冲突(60.58%)。7个模块35项卫勤保障能力内容的实际培训率为51.82%~99.27%,培训需求率为86.50%~93.61%,其中手术护理模块全部内容、骨髓腔穿刺技术和微光/动态下静脉穿刺技术培训率最低,实际培训经历与培训需求差异较大。结论军队护士参加野战医疗所部分职能内容的培训率高但完成全流程职能内容的培训率有待进一步提升;护士对相关内容的培训需求较大但工作与训练矛盾突出;培训及考核等形式多样但更应贴近实战与护士需求。医院应以培养一专多能护理卫勤队伍为目标,建立基于野战医疗所开展全流程职能的护理卫勤保障能力培训模式及考核机制,以促进培训的同质化与规范化。
Objective To understand the current situation and demand of medical support ability training for military nurses in the whole process nursing post in field medical station,and to provide basis for formulating scientific and reasonable training mode and training assessment mechanism.Methods From August to September 2022,nurses from 2-3 military hospitals in each of the five war zones were selected by convenience sampling method to conduct self-designed questionnaire survey through the questionnaire Star platform.Results A total of 548 military nurses participated in the survey,57.85%of them had work experience in field medical station,55.66%had participated in rotation training,and 26.28%had training experience in 7 modules and 35 items of health sevice support capability related to the whole process of field medical station.The actual training rate of 35 items was 51.82%~99.27%,and the training demand rate was 86.50%~93.61%.Among them,the training rate of all contents of surgical nursing,bone marrow puncture technology and low-light/dynamic venipentesis technology was the lowest,and the training experience and training demand were significantly different.Hospital public training(59.85%)and special task training(41.97%)were the main ways of nurses′health service training;the main forms of training included theoretical teaching(91.79%),operational practice(88.50%)and situational simulation training(55.66%);part-time study(41.24%)was the main time;the main assessment forms were theoretical assessment(76.28%)and operational assessment(84.49%);82.66%of nurses believed that there were problems in the existing training of nursing and health service,mainly due to the conflict between work and training time(60.58%).The actual training rate of 35 items related health sevice support capability in the 7 modules was 51.82%~99.27%,and the training demand rate was 86.50%~93.61%,among which the training rate of the whole content of surgical nursing module,marrow cavity puncture technology and low-light/dynamic venipentesis technology was the lowest,and the actual training experience and training demand were significantly different.Conclusion The training rate of military nurses to participate in some functional content of field medical station is high,but the training rate to complete the whole process of functional content needs to be further improved;nurses have a great demand for training related content,but the contradiction of training is prominent;there are various forms of training and assessment,but they should be closer to actual combat and nurses′needs.Aiming at cultivating a multi-functional nursing and health service team,the training mode and assessment mechanism of nursing and health service support ability based on the whole process function of field medical station should be established to promote the homogenization and standardization of training.
