中药湿敷和中药熏洗坐浴对痔疮术后患者创面愈合及疼痛的影响

Effect of Chinese medicine wet compress and Chinese medicine fumigation on wound healing and pain in patients with hemorrhoids after surgery

目的分析痔疮术后患者配合以中药湿敷和中药熏洗坐浴干预对促进创面愈合及疼痛的影响。方法选取厦门市中医院2023年1月—12月的170例痔疮术后患者,采用随机数字表法分为对照组与观察组,各85例。对照组患者采用常规护理,观察组患者在对照组基础上实施中药湿敷和中药熏洗坐浴。对比两组术后疼痛程度、症状恢复指标(首次排便时间、肛门坠胀消失时间和创伤愈合时间)、水肿程度、创面评分和睡眠质量(总睡眠时间、睡眠潜伏期和睡眠效率)。结果术后1 h,两组VAS评分对比无差异(P>0.05)。术后24 h和48 h,观察组VAS评分均低于对照组(P<0.05)。观察组首次排便时间、肛门坠胀消失时间和创伤愈合时间均短于对照组(P<0.05)。术后48 h,观察组水肿程度和创面评分低于对照组(P<0.05)。护理后,观察组总睡眠时间、睡眠潜伏期和睡眠效率均优于对照组(P<0.05)。结论中药湿敷和中药熏洗坐浴联合临床护理能有效减轻痔疮术患者术后疼痛程度,促进痔疮术后患者创面愈合,并有利于提高患者睡眠质量,缩短痔疮术患者术后康复时间,改善患者水肿情况。

Objective To analyze the impact of traditional Chinese medicine(TCM)wet compression and TCM fumigation and washing sitz baths on promoting wound healing and pain relief in patients after hemorrhoid surgery.Methods A total of 170 patients who underwent hemorrhoid surgery at Xiamen Traditional Chinese Medicine Hospital from January 2023 to December 2023 were selected and randomly divided into a control group and an observation group,each consisting of 85 cases.The control group received routine nursing care,while the observation group,in addition to the control group’s care,underwent TCM wet compression and TCM fumigation and washing sitz baths.The postoperative pain levels,symptom recovery indicators(time to first defecation,disappearance of anal fullness time,and wound healing time),edema degree,wound score,and sleep quality(total sleep time,sleep latency,and sleep efficiency)were compared between the two groups.Results There was no difference in VAS scores between the two groups 1 hour after surgery(P>0.05).At 24 and 48 hours postoperatively,the VAS scores in the observation group were lower than those in the control group(P<0.05).The time to first defecation,disappearance of anal fullness,and wound healing time in the observation group were all shorter than in the control group(P<0.05).At 48 hours postoperatively,the edema degree and wound score in the observation group were lower than in the control group(P<0.05).After nursing care,the total sleep time,sleep latency,and sleep efficiency in the observation group were all better than in the control group(P<0.05).Conclusions The combination of TCM wet compression and TCM fumigation and washing sitz baths with clinical nursing can effectively reduce the degree of postoperative pain in patients after hemorrhoid surgery,promote wound healing,improve sleep quality,shorten the recovery time after surgery,and improve edema conditions.
