冠脉内旋磨术治疗稳定型心绞痛前降支钙化病变患者的临床效果

Clinical Effect of Coronary Internal Grinding in the Treatment of Stable Angina Pectoris with Anterior Descending Branch Calcification

目的:探讨冠脉内旋磨术治疗稳定型心绞痛前降支钙化病变患者的临床效果。方法:回顾性选择2021年1月—2023年2月龙岩市第一医院收治的90例稳定型心绞痛前降支钙化病变患者,依据治疗方案不同将其分为两组,45例患者采取球囊扩张+支架植入术,纳入对照组,45例于对照组治疗基础上应用冠脉内旋磨术,纳入观察组,比较两组患者手术效果、心功能变化及心绞痛复发情况。结果:相比于对照组,观察组术后最小管腔直径更大,病变最重狭窄率更低,差异有统计学意义(P<0.05);术后6个月,相比于对照组,观察组左室射血分数更高,左心室舒张末期内径更小,差异有统计学意义(P<0.05);观察组术后1年内心绞痛的复发率较对照组低,差异有统计学意义(P<0.05)。结论:冠脉内旋磨术在稳定型心绞痛前降支钙化病变患者中的应用,能够显著改善患者的心功能,提高手术效果,降低心绞痛的复发率。

Objective:To investigate the clinical effect of coronary internal grinding in the treatment of stable angina pectoris with anterior descending branch calcification.Method:A total of 90 patients with anterior descending branch calcification of stable angina pectoris treated in the First Hospital of Longyan from January 2021 to February 2023 were retrospectively selected,and divided into two groups according to different treatment plans.45 patients were included in the control group by balloon dilation plus stent implantation,and 45 patients were included in the observation group by coronary internal grinding based on the treatment of the control group.The operative effect,cardiac function changes and recurrent angina pectoris were compared between the two groups.Result:Compared with the control group,the observation group had a larger minimum lumen diameter and a lower rate of heaviest stenosis,the differences were statistically significant(P<0.05).At 6 months after operation,compared with control group,left ventricular ejection fraction was higher and left ventricular end-diastolic diameter was lower in observation group,the differences were statistically significant(P<0.05).The recurrence rate of angina pectoris in the observation group was lower than that in the control group with in one year after operation,and the difference was statistically significant(P<0.05).Conclusion:The application of coronary internal grinding in patients with stable anterior descending branch calcification of angina pectoris can significantly improve the cardiac function of patients,improve the operative effect,reduce the recurrence rate of angina.
