38例肺癌的CT引导下微波消融安全性及初步疗效临床总结

Clinical Summary of Safety and Efficacy of CT-guided Microwave Ablation in 38 Cases of Lung Cancer

摘要 目的:探讨CT引导下微波消融(microwave ablation,MWA)在肺癌治疗中的安全性及近期疗效。方法:选取2023年1月1日至2023年12月31日在联勤保障部队第九六二医院胸外科行CT引导下MWA治疗的38例肺癌患者病历资料,其中男性31例(81.6%),平均年龄44.2±14.3岁,肿瘤平均直径13.1±5.5mm,肿瘤位于右肺上叶9例(23.7%),右肺中叶7例(18.4%),右肺下叶6例(15.8%),左肺上叶10例(26.3%),左肺下叶6例(15.8%),回顾性分析临床资料、消融成功率、近期疗效及并发症情况,评价CT引导下微波消融在肺癌治疗中的安全性及有效性。结果:38例肺肿瘤共进行了38次消融治疗,均达到完全消融,消融成功率100%,术后平均随访10.2±2.9个月,随访期间无复发。影像学病灶转归为,纤维化29例(76.3%),病灶完全消失7例(18.4%),空洞2例(5.3%)。最常见的不良反应是疼痛,共有18例(47.4%);最常见的并发症为气胸,38例患者中共出现12例(31.6%),其中11例为局限性气胸,仅需临床观察即可。仅出现1例严重并发症为大量气胸,经胸腔闭式引流治愈,无死亡病例。结论:CT引导下MWA是一种较有效的肺癌局部治疗手段,具有较好的安全性和一定疗效。 Objective:The purpose of this study was to investigate the safety and short-term efficacy of CT-guided microwave ablation(MWA)in the treatment of lung cancer.Methods:The data of 38 patients with lung cancer who underwent CT-guided MWA treatment in the Department of Thoracic Surgery of the 962 Hospital of the Joint Logistics Support Force from January 1,2023 to December 31,2023 were selected.Among them,31(81.6%)were males,with an average age of 44.2±14.3 years and an average tumor size of 13.1±5.5 mm.Tumors were located in the right upper lobe in 9 cases(23.7%),the right middle lobe in 7 cases(18.4%),the right lower lobe in 6 cases(15.8%),the left upper lobe in 10 cases(26.3%),and the left lower lobe in 6 cases(15.8%).The clinical data,complete ablation rate,short-term efficacy and complications were retrospectively analyzed to evaluate the safety and short-term efficacy of CT-guided MWA in the treatment of lung cancer.Results:A total of 38 pulmonary tumors were ablated for 38 times,all of which achieved complete ablation with a success rate of 100%.The average postoperative follow-up was 10.2±2.9 months,and there was no recurrence during the follow-up.The imaging outcomes was fibrosis in 29 cases(76.3%),complete disappearance in 7 cases(18.4%),and cavity in 2 cases(5.3%).The most common adverse reaction was pain(18 cases(47.4%).The most common complication was pneumothorax,which occurred in 12 of 38 patients(31.6%),of which 11 were localized pneumothorax,requiring only clinical observation.Only one serious complication was massive pneumothorax,which was cured by closed thoracic drainage,and no death occurred.Conclusion:CT-guided MWA is an effective local treatment for lung cancer with good safety and short-term efficacy.
