7S管理模式对初产妇哺乳技能、母乳喂养信心及产后开始泌乳时间的影响

Impact of 7S management model for primiparas on their lactation skills and breastfeeding confidence,and the time to start lactation after childbirth

目的:分析7S管理模式对初产妇哺乳技能、母乳喂养信心及产后开始泌乳时间的影响。方法:选取2021年10月-2022年10月本院收治的待产初产孕妇108例,按照随机数字表法分为常规组53例、7S管理组55例,分别给予常规干预或7S管理模式。评估对比两组哺乳技能、母乳喂养信心、产后康复情况、心理弹性、主观幸福感、母亲角色适应能力、心理状态、护理满意度。结果:护理干预后,7S管理组与常规组哺乳技能(29.88±3.62分、26.22±3.42分)、喂养情感(66.72±6.01分、60.29±5.88分)、喂养认知度(26.01±3.11分、22.31±2.58分)、心理弹性、主观幸福感、母亲角色适应能力评分均上升,且7S管理组高于常规组;两组心理状态评分均下降,且7S管理组低于常规组;7S管理组住院时间(3.20±0.90d)、血性恶露持续、开始泌乳时间(15.30±3.31h)、子宫入盆时间均短于常规组(5.36±1.02d、20.60±4.02h),护理满意度(94.6%)高于常规组(81.1%)(均P<0.05)。结论:初产妇采用7S管理模式干预后哺乳技能、母乳喂养信心提升,开始泌乳时间缩短,心理状态改善,心理弹性、主观幸福感、母亲角色适应能力增强。

Objective:To analyze the impact of 7S management model for primiparas on their lactation skills,their breastfeeding confidence and their time to start lactation after childbirth.Methods:1o8 primiparas who admitted to the hospital for delivery were selected and were randomly divided into control group(53 cases)and study group(55 cases)from October 2021 to October 2022.The primiparas in the control group received the routine intervention,and the primiparas in the study group received 7S management model.The breastfeeding skills,the breastfeeding confidence,the postpartum recovery situation,the psychological resilience,the subjective well-being,the maternal role adaptation,the psychological status and the nursing satisfaction of the primiparas were evaluated and compared between the two groups.Results:After the intervention,the scores of the breastfeeding skills,the breastfeeding and the breastfeeding cognition of the primiparas in the two groups had increased significantly,and which(29.88±3.62 points,66.72±6.01 points and 26.01±3.11 points)of the primiparas in the study group were significantly higher than those(26.22±3.42 points,60.29±5.88 points and 22.31±2.58 points)of the primiparas in the control group.After intervention,the scores of the psychological resilience,the subjective well-being and the mother role adaptation of the primiparas in the two groups had increased significantly,and which of the primiparas in the study group were significantly higher than those of the primiparas in the control group.The psychological status score of the primiparas in the two groups after intervention had decreased significantly,and which of the primiparas in the study group was significantly lower than that of the primiparas in the control group.The hospital stay(3.20±0.90d)and the beginning time of lactation(15.30±3.31h)of the primiparas in the study group were significantly shorter than those(5.36±1.02d and 20.60±4.02h)of the primiparas in the control group.The duration of the bloody lochia and the time of the uterus returned into the pelvis of the primiparas in the study group were significantly shorter than those of the primiparas in the control group.The nursing satisfaction(94.6%)of the primiparas in the study group was significantly higher than that(81.1%)of the primiparas in the control group(P<0.05).Conclusion:The 7S management mode intervention for the primiparas can improve their breastfeeding skills and confidence,can shorten their time to start lactation,can improve their psychological status,and can enhance their psychological resilience,subjective well-being and maternal role adaptability.
