前列腺增生合并输尿管结石患者应用钬激光碎石术治疗的效果

Effect of Holmium Laser Lithotripsy on Patients with Prostatic Hyperplasia Complicated with Ureteral Calculi

目的:观察输尿管镜下钬激光碎石术治疗前列腺增生合并输尿管结石的临床效果。方法:筛选2022年3月—2024年4月宣城市泾县医院收治的84例前列腺增生合并输尿管结石患者,随机分为对照组(应用腹腔镜手术治疗)和观察组(应用输尿管镜下钬激光碎石术治疗),各42例。比较两组临床相关指标情况。结果:观察组腰痛、尿路梗阻持续时间及住院时间短于对照组,满意度评分高于对照组(P<0.05);治疗后,两组国际勃起功能指数-5评分、射精功能评分、国际前列腺症状评分、生活质量评分优于治疗前,观察组优于对照组(P<0.05);治疗后,两组最大尿流量、膀胱残余尿量、膀胱最大容量优于治疗前,观察组优于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.001)。结论:应用输尿管镜下钬激光碎石术治疗前列腺增生合并输尿管结石,能够控制患者的前列腺症状,改善性功能及尿动力学指标,提高患者生活质量、满意度,安全性较高。

Objective:To observe the clinical effect of holmium laser lithotripsy under ureteroscope in the treatment of prostatic hyperplasia complicated with ureteral calculi.Methods:A total of eighty-four patients with prostatic hyperplasia complicated with ureteral calculi admitted to Xuancheng Jingxian County Hospital Province from March 2022 to April 2024 were selected and randomly divided into control group(treated with laparoscopic surgery)and observation group(treated with ureteroscopic holmium laser lithotripsy),with forty-two cases in each group.The clinical correlation indexes of the two groups were compared.Results:The duration of low back pain,urinary obstruction and hospital stay in the observation group were shorter than those in the control group,and the satisfaction scores were higher than those in the control group(P<0.05);after treatment,the scores of international erectile function index-5,ejaculation function score,international prostate symptom score and quality of life score of the two groups were better than before treatment,and the observation group was better than the control group(P<0.05);after treatment,the maximum urine flow,bladder residual urine volume and bladder capacity of the two groups were better than before treatment,and the observation group was better than the control group(P<0.05);the complication rate of the observation group was lower than that of the control group(P=0.001).Conclusion:The application of ureteroscopic holmium laser lithotripsy in the treatment of prostatic hyperplasia complicated with ureteral calculi can control prostate symptoms,improve sexual function and urodynamic indexes,improve patients'quality of life,satisfaction and safety.
