社区-综合医院全科医师分级管理老年多病共存的效果分析

Analysis of the Effect of Graded Management of Elderly Patients with Multiple Diseases by General Practitioners in Community Comprehensive Hospitals

分析社区-综合医院全科医师分级管理在老年多病共存管理中的应用效果。以200例多病共存老年患者为研究对象(2023年3月—2024年3月),利用随机数字表法将其分为观察组与对照组,对照组实施常规管理,观察组实施社区-综合医院全科医师分级管理,对比两种管理模式的应用效果。结果显示,管理后,两组患者的生存质量评分均升高,且观察组高于对照组(P<0.05)。管理后,两组患者的各项自护能力评分均升高,且观察组高于对照组(P<0.05)。观察组患者的管理效果优于对照组(P<0.05)。研究发现,社区-综合医院全科医师分级管理在老年多病共存管理中的应用效果显著,能够提高患者的生活质量,增强患者的自护能力,节省患者的治疗费用,降低患者的再入院率,值得临床推广。

Analyze the application effect of community comprehensive hospital general practitioner grading management in the management of elderly patients with multiple diseases.200 elderly patients with multiple comorbidities were selected as the research subjects(March 2023 March 2024),and were randomly divided into an observation group and a control group using a random number table method.The control group received routine management,while the observation group received graded management from community general practitioners in a comprehensive hospital.The application effects of the two management models were compared.The results showed that after management,the quality of life scores of both groups of patients increased,and the observation group was higher than the control group(P<0.05).After management,the self-care ability scores of both groups of patients increased,and the observation group was higher than the control group(P<0.05).The management effect of the observation group patients was better than that of the control group(P<0.05).Research has found that the application effect of graded management of general practitioners in community comprehensive hospitals in the management of elderly patients with multiple diseases is significant,which can improve patients’quality of life,enhance their self-care ability,save patients’treatment costs,and reduce their readmission rate.It is worthy of clinical promotion.
