摘要
目的探讨Cawthorne-Cooksey前庭康复训练方法在突发性耳聋伴眩晕患者中的应用价值。方法选取2022年6月—2024年3月重庆医科大学附属永川医院收治的80例突发性耳聋伴眩晕患者为研究对象,按照随机数字表法将其分为对照组和试验组,各40例。对照组采用常规药物治疗,试验组在对照组基础上采用Cawthorne-Cooksey前庭康复训练治疗,两组均持续治疗2周。比较两组的临床疗效、听力水平、眩晕程度及平衡能力。结果两组治疗总有效率比较,差异无统计学意义(P>0.05)。治疗后,两组平均听阈均值均低于治疗前,差异有统计学意义(P<0.05),但两组平均听阈值比较,差异无统计学意义(P>0.05);试验组眩晕残障程度评定量表评分为(26.87±9.36)分,低于对照组的(32.92±11.63)分,差异有统计学意义(P<0.05);试验组Berg平衡量表评分为(43.98±4.08)分,高于对照组的(41.15±5.09)分,差异有统计学意义(P<0.05)。结论突发性耳聋伴眩晕患者采用常规药物治疗联合Cawthorne-Cooksey前庭康复训练可以改善患者的眩晕症状,恢复平衡能力,但临床听力恢复效果有限。
Objective To explore the application value of Cawthorne-Cooksey vestibular rehabilitation training in patients with sudden deafness and vertigo.Methods Eighty patients with sudden deafness and vertigo admitted to Yongchuan Hospital Affiliated to Chongqing Medical University from June 2022 to March 2024 were selected as the research objects,and were divided into a control group and an experimental group according to random number table method,with 40 cases in each group.The control group was treated with conventional drugs,and the experimental group was treated with Cawthorne-Cooksey vestibular rehabilitation training on the basis of the control group,both groups were treated for 2 weeks.The clinical efficacy,hearing level,vertigo degree and balance ability of the two groups were compared.Results There was no significant difference in the total effective rate between the two groups(P>0.05).After treatment,the average hearing threshold between the two groups were lower than those before treatment,the differences were statistically significant(P<0.05),However,when comparing the average hearing thresholds between the two groups,the differences were no statistically significant(P>0.05);the score of Dizziness Handicap Inventory scale in experimental group was(26.87±9.36)points,which was lower than(32.92±11.63)points in control group,and the difference was statistically significant(P<0.05);the score of Berg Balance Scale in the experimental group was(43.98±4.08)points,which was higher than(41.15±5.09)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Cawthorne-Cooksey vestibular rehabilitation training can improve vertigo symptoms and restore balance in patients with sudden deafness and vertigo,but the clinical effect of hearing recovery is limited.
