5-氟尿嘧啶与小剂量顺铂联合治疗晚期消化道肿瘤的临床观察

Clinical Observation on Low Dose Cisplatinum (DDP) Combined with 5-Fluorouracil for Late Castrointestinal Carcinoma

目的 观察 5 -氟尿嘧啶与小剂量顺铂联合治疗晚期消化道肿瘤的疗效及毒副作用。方法 将 1998年 9月～ 2 0 0 0年 8月我科住院消化道肿瘤病人随机分为两组 :实验组应用 5 -Fu +小剂量顺铂 ,不水化。对照组用 5 -Fu +大剂量顺铂 ,辅助水化、利尿、止吐及升白治疗。结果 实验组有效率 (CR +PR) 5 9 5 % ,其中 3例获CR ,对照组有效率 (CR +PR) 3 2 0 % ,有 2例获CR ,实验组恶心、呕吐、白细胞下降等发生率低于对照组 (P <0 0 5 )。结论 ① 5 -氟尿嘧啶与小剂量顺铂联合应用时存在生物化学相互修饰的作用 ,使两药疗效提高。② 5 -氟嘧啶与小剂量顺铂联合化疗毒副作用小 ,病人易于接受 。

Objective To observe the effects and sideeffects of low dose cisplatinum (DDP) combined with 5-fluorouracil(5-Fu) for late gastrointestinal carcinoma.Methods From september 1998 to August 2000,the patients in our hospital who had late gastrointestinal carcinoma were randomly divided into two groups;the testing group,with the low dose of DDP combined with 5-Fu regimen,no watering therapy,the control group,with the large dose of DDP combined with 5-Fu regimen,assisting watering therapy,ceasing nausea and vomiting,and increasing white blood cell.Results In the testing group,the overall response rate (CR+PR) was 59.5%,and three patients had complete response.In the control group the overall response rate (CR+PR) was 32.0%,and two patients had complete response ( P <0 05).Nausea,vomiting,leakopenia,etc in the testing group had happened less often than those in the control group ( P <0.05).Conclusion ①When low dose DDP is combined with 5-Fu,they ineract by biochemical modulation,which increases the effects of the drugs.②The low dose DDP combined with 5-Fu regimen for late gastrointestinal carcinoma has least toxicity and side-effects,so the patients can tolerate and receive it.The regimen is worth utilizing extensively.
