超声背向散射积分和谐波技术在老年慢性肾功能不全临床诊断中的应用 被引量:1

摘要 目的 :与彩色、能量Doppler对比 ,探讨超声背向散射积分 (IBS)和对比二次谐波 (SHCI)技术在定量诊断老年慢性肾功能不全方面的应用价值。方法 :用HP 5 5 0 0型超声仪检测 5 5例老年男性患者 ,其中肾功能正常者32例 (取 40个肾脏 ) ,慢性肾功能不全 2 3例 (取 40个肾脏 ) ,进行Doppler(彩色和能量 )、IBS、SHCI、能量SHCI(PSHCI)和声学密度定量 (AD)等检查比较。结果 :①肾功不全组较正常者 :AD峰值和曲线下面积显著增高 (P <0 0 5 ) ,IBS、SHCI回声明显增强 ;PSHCI视觉判断肾皮、髓质血流灌注显著减少 ;②IBS、SHCI对肾皮、髓质识别更加清楚 ,PSHCI使流速较低的皮质外层边缘区灌注显像。结论 :IBS、SHCI、PSHCI和AD技术提高了超声分辨率、改善了成像质量、降低了人为因素差异 ,使超声组织定征在无创诊断老年肾功能改变方面具有重要的临床应用价值。 Objective:To assess whether IBS and SHCI would improve renal ultrasound imaging for evaluating renal function.Methods: In 55 old male patients, IBS and SHCI were performed in 40 normal kidneys and 40 kidneys with renal inadequacy compared by quantitative assessment of Acoustic Densitometry(AD).Results: the peak intensity,the area under the curve were significantly higher than the control group (both P<0.05). Conclusion: The results showed that IBS and SHCI were a safe,efficient new technology for assessing renal function different from the Doppler Ultrasound, improes ultrasound images .The recent availabity of AD helps to distinguish between normal and abnormal renal function, and should help to reduce operator variability.
