摘要
目的 分析肌肉淋巴瘤的CT表现 ,了解其影像特点。资料与方法 回顾性分析经组织学或临床随诊证实的 11例肌肉淋巴瘤的CT表现。非霍奇金淋巴瘤 (NHL) 9例 ,霍奇金淋巴瘤 (HD) 2例。 9例行CT增强扫描 ,2例行CT平扫。结果 1例为单块肌肉受累 ,其余 10例均为多处肌肉同时受累 ,其中 8例为多处相连肌肉受累 ,2例为多处不相连肌肉受累。受累部位 :髂腰肌 7例 ,腰大肌 5例 ,髂肌 4例 ,腰方肌、闭孔内肌、腹壁肌肉各 3例 ,肋间肌、背阔肌、梨状肌、臀肌各 2例 ,胸肌、胸锁乳突肌各 1例。病变形态 :肌肉弥漫增大 11例 ,肿块、多个结节融合各 1例。平扫 2例 ,病变密度与正常肌肉相似 ,1例均匀 ,另 1例不均匀 ,内部见不规则片状低密度区。增强扫描 9例 ,病变密度等于正常肌肉 7例 ,轻度高于正常肌肉 3例 ,明显高于正常肌肉 1例 ;密度均匀 4例 ,不均匀 6例 ;病变内部见不规则片状低密度区 6例 ,点状钙化 1例。结论 肌肉淋巴瘤的CT表现特点多为肌肉弥漫增大 ,常多处肌肉同时受累。平扫密度与正常肌肉相似 ,增强扫描可等于。
Objective To study CT manifestations of muscle lymphoma. Materials and Methods CT findings in 11 patients with clinically or pathologically proved muscle lymphoma, including non Hodgkin lymphoma (n=9) and Hodgkin lymphoma (n=2), were retrospectively analyzed. Enhanced CT scanning was performed in 9 patients and plain CT scanning in 2.Results A single muscle involvement was seen in one case, while coexisting musclular involvement at multiple sites in 10 cases, of which the affected muscles were continuous with each other in 8, and non continuous with each other in 2. The most affected sites included iliopsoas (n=7), psoas (n=5), and iliacus (n=4). CT findings included diffuse muscle enlargement (n=11), local mass (n=1) and fused multinodular lesion (n=1). On plain CT scans (n=2), the lesion's density was similar to that of normal muscle, and was homogeneous (n=1) or heterogeneous (n=1). On enhanced CT scans (n=9), the lesion was manifested as iso density (n=7), slightly high density (n=3) or markedly high density (n=1), being homogeneous (n=4) or heterogeneous (n=6). Irregular patch shaped hypodense area within the lesion was found in 6 cases, and spot calcification in one.Conclusion On CT scans, muscle lymphoma is characterized by diffuse muscle enlargement and coexisting muscular involvement at multiple sites. The lesion exhibits the density similar to the normal muscle on plain CT scan, and shows iso density or slight to marked high density on enhanced CT scan.
